Confirmatory Action Letter I-91-012 - Site Area Emergency at Nine Mile Point Unit 2 Pursuant to the telephone conversation between Mr. William Kane, Deputy Regional Administrator and Mr. Joseph Firlit, of your staff, on August 13, 1991, it is our understanding that you have taken or will take the following actions:
1.
Maintain appropriate records related to the event which occurred on August 13, 1991, and preserve any evidence, including records of as-found equipment conditions, 'which would be needed to inspect or reconstruct the event.
2.
Assure that. the Augmented Inspection Team (AIT)leader is informed in advance ofyour planned and on-going actions to investigate and evaluate any equipment.
3.
Perform a root cause analysis of this event.
This analysis will include the underlying causes for the event, an assessment oftheir relationship to previous events; and corrective actions taken and/or planned.
Provide a briefing to NRC Region I management prior to restart that contains your evaluation of the findings and recommendations of the root cause analysis and provides your basis for restart, including the restart criteria used and your analyses to satisfy these criteria.
Further, we understand that restart willnot occur until you receive agreement from the Regional Administrator, Region I.
The response directed by this letter is not subject to the clearance procedures of the Office of Management and Budget, as required by the Paperwork Reduction Act of 1980, PL 96-511.
