SUMMARY

This report is written to provide an analysis and interpretation of the results of the Type A test. The subject test was performed during the first refueling outage.

Pressurization of the containment began at 2300 on January 4, 1991. Final containment test pressure was reached at 0915 on January 5, 1991. The test was conducted from 1315 to 2115 on the same date. The verification test was completed on January 6,1991 at 0230. Depressurization was completed at 0330 on January 7, 1991.

The acceptance criteria established for the test are found in 10CFR50 Appendix J, Section III A.5.b(2) and Nine Mile Point Unit 2 Technical Specification 3.6.1.2, These values are less than 0.75 La, or 0.825 %/day and less than or equal to 1.1 %/day respectively. The Type A "as found" results were 0.623 %/day. This number includes penalties and "as found" vs. "as left" savings form the Type B and C tests. The "as left" result was 0.305 %/day. The "as left" result also includes appropriate 8 and C penalties.

Based on the above it can be seen that both "as found" and "as left" results are below the acceptance criteria.

The report also includes a summary of Type B and C tests performed since the last Type A test. This summary is provided in appendix A. Appendix B provides an analysis of the failures and gives descriptions of corrective actions performed.

1.0 INTRODUCTION

In accordance with the requirements of 10CFR50 Appendix J, Section V, Paragraphs B.1 and B.3, this report provides the results of the Nine Mile Point Nuclear Station, Unit 2, 1991 Integrated Leak Rate Test. The general form and content of this report was written in accordance with ANSI/ANS-56.8-1987, "Containment System Leakage Testing Requirements", Section 5.8.

6.0 ANALYSIS AND INTERPRETATION 6.1 Acce ance Criteria Based on Nine Mile Unit 2 Technical Specification Limiting Condition for Operation 3.6.1.2, the primary containment overall integrated leakage rate shall be maintained less than or equal to La, or 1 1% by weight of the

This was interpreted to mean that the upper confidence limit on the measured leakage rate (UCL), including all Type B and C corrections, penalties, and other leakage additions, shall be less than the above acceptance criteria.

Where; Lo = Superimposed Leakage rate which is between 75 and 125% of La, Lam = Last valid Type A measured/calculated leakage rate, Lc = Composite leakage rate as measured by the data acquisition system during the verification test, La = Maximum allowable leakage rate at pressure Pac.

6.2 Instrumentation S stem The test instrumentation system consisted of: (1) a microprocessor controlled data acquisition system (DAS); (2) twenty-two drybulb resistance temperature detectors; (3) six dew point temperature sensors; and (4) two precision pressure manometers. For testing purposes, only one precision pressure manometer connected to the drywell was used . Prior to containment pressurization, the calculated instrumentation system Instrument Selection Guide (ISG) was 0.0228%/day for an assumed eight hour test duration. This value was below the required value of 0.275%/day.

The instrumentation system configuration at the beginning and end of the test was:

In accordance with 10CFR50 Appendix J, Section V.B.3, a summary analysis of periodic Type 8 tests performed since the last Type A test is provided in appendices A and B.

6.4 T e C Air Test Resul s (Ref. Appendices A and B)

In accordance with 10CFR50 Appendix J, Section V,B.3, a summary analysis of periodic Type C tests performed since the last Type A test is provided in appendices A and B.

6.5 T e B and C Pretes Penal Leaka e (Ref. Appendix C)

In accordance with 10CFR50 Appendix J, Section III.A.1.d, certain systems penetrating the containment boundary were not vented and drained. In order to compute the appropriate Type A Test penalty to be applied to the upper confidence limit, an analysis of each affected penetration was performed, based on minimum path analysis as described in the Nine Mile Point Unit 2 USAR. Based on this analysis, a total pretest penalty leakage of 0.017 %/day was assigned. Appendix C is a detailed summary of these penetrations and their assigned penalty leakages based on Type B and Type C Test results.

6.6 S ruc ural lnte ri Ins ec ion and Evalua ion In accordance with 10CFR50 Appendix J, Section V.A, the primary containment was inspected for structural integrity. During this inspection, two conditions requiring additional engineering evaluation were observed.

1) During the pretest inspection by Maintenance Engineering, it was discovered that a number of piping plugs (located on the penetration test head) were missing from'four "Z" type penetrations. The intended function of the penetration test heads is to provide a means to perform a Positive Pressure Decay Test of welds between the liner insert plates and the penetration sleeves. The test heads do not comprise part of the primary containment liner boundary. Based on engineering evaluation, it was determined that the missing plugs were NMP2-ILRT 4of 18

Documentation related to the engineering evaluation of this condition is available at the plant site.

2) During the pretest inspection by Maintenance Engineering, it was discovered that the paint coating on many weld joints was cracking, peeling, and flaking. The unsatisfactory loose paint condition has been determined to be a specified amount of unqualified coating within the containment area. Based on engineering evaluation, it was determined that the affected coating conditions had been previously accounted for in other analyses, Documentation related to the engineering evaluation of this condition is available at the plant site.

6.7 Area Tem era ure Surv An area temperature survey of the primary containment was performed to verify that the instrument locations and associated volume fractions employed during the preoperational test were valid for the first Unit 2 inservice test. The following conditions were established prior to the survey:

1. All Drywell fans secured and cooling water isolated.
2. Recirculation System secured.
3. Loop B of the Residual Heat Removal System in service.
4. Reactor coolant temperature controlled between 102 'F and 108 'F.

The results of the survey confirmed the correct location of the dry bulb and dew point temperature sensors for the containment ventilation configuration employed during the test.

The containment atmosphere was allowed to stabilize for 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br />. The latest rate of drybulb temperature change over the last hour of the stabilization period was 0.022'F. The rate of change over the last four hours of the stabilization period was 0.185'F, indicating a stable containment atmosphere. Further, a plot of containment air mass versus time was observed to be trending towards linear behavior, thus confirming containment atmospheric stability. (See Figure 6.8-1.)

6.9 As Le Containmen T e A Tes The Type A Test was begun at 1315 hours0.0152 days <br />0.365 hours <br />0.00217 weeks <br />5.003575e-4 months <br /> on January 5, 1991. Based on the Total Time Analysis Method, the test was declared over at 2115 on January 5, 1991. The Type A Test results must be corrected for Type B and Type C test penalty leakages, which are equal to 0.017 %/day. (See Appendix C.) The following table summarizes the as left test results.

(See Figure 6.9-2.) Based on the above data, it was concluded that the as left containment leakage rate was stable, below the acceptance criteria, and therefore the containment may be declared operable in accordance with the plant Technical Specifications, Also see Appendix D for a tabulation of Type A test results.

6.10 As Found Containmen T e A Tes Type A Test results must be compensated for the "as found" vs. "as left" leakages for all penetrations if the Type B and C Tests are conducted prior to the Type A Test. These as found vs. as left leakages were tabulated based on the minimum pathway leakage in Procedure N2-TSP-CNT-@003 and were equal to 0.006 %/day. The following table summarizes the as found test results.

Based on the above data, it was concluded that the composite leakage rate NMP2-ILRT 8 of 18

NIAGARA MOHAWK POWER CORPORATION NINE MILE POINT UNIT 2 ILRT FINAL REPORT was stable, within the limits of the test, and therefore, the Type A Test instrumentation system was considered to be verified functional throughout the Type A Test. Also see Appendix E for a tabulation of verification test results.

6.12 Con ainmen Penetra ion Re ir rA 'en No repairs or adjustments were made to the containment boundary during the Type A Test or the subsequent verification test.

During containment pressurization, a thorough walkdown of all accessible penetrations was conducted. During this walkdown, two conditions were identified that required adjustment to the containment boundary.

1) RCIC turbine exhaust line check valve V29 was leaking suppression pool water to the RCIC turbine. In order to prevent damage to the turbine and contamination of its oil systems, motor operated valve MOV"122 was closed, effectively isolating penetration Z-19 and stopping the leak. The minimum pathway leakage for this penetration is .742 SCFH. Therefore, this value was included in Appendix C, Pre Test Penalty Leakage Results as a valve lineup change penalty.
2) Containment pressure transmitter 2RCS-PT84A was observed to be leaking through the high side bleed and vent valve. This valve was considered to be part of the containment boundary valve alignment by the test director and as such was ordered closed by normal means using l&C craft personnel. However, upon further investigation, it was discovered that the condition of this valve may have existed for a significant period of time prior to the Type A test. Since no local leakage rate test results were available for this condition, the contribution of this valve to the as found containment condition is indeterminate. The maximum leakage through this valve would occur just prior to the checking setpoint of the instrument excess flow check valve. The setpoint for this valve is 4.0 SCFIVI. Therefore, this value was used to compensate the as found test results for this condition. These results are presented in Table 6.10-1.

'UMMARY FAILURE TO MEET ACCEPTANCE CRITERIA OF 10CFR50 APPENDIX J SECTION III.C.3.a The acceptance criteria specified in 10CFR50 Appendix J, Section III.C.3.a was exceeded during the October 1988 outage and the First Refueling outage. During these outages the "as found" maximum pathway total for Type B and Type C tests exceeded 0.6 La. A description of these events is provided below.

0 ober 1988 Ou a e During this outage valve 2CSH "AOV108 was found to be leaking excessively.

Efforts to quantify the leakage resulted in a determination that the valve leakage was 205.194 SCFH. This was added to the Type B and C total leakage calculated in procedure N2-ISP-CNT-R003. It was discovered that this leakage resulted in the maximum pathway total exceeding the Appendix J Acceptance Criteria of 0.6 La. A review of test results indicates that no other single valve would have caused the 0.6 La limit to have been exceeded. Additionally the results of a minimum pathway analysis yielded values less than La. The subject valve was repaired (see Appendix C,Section III, Item 10). Prior to restarting the plant from the outage it was determined, in accordance with N2-ISP-CNT-R003, that the "as left" maximum pathway leakage was below 0.6 La.

First Refuelin Ou a e On 9/17/90, during routine Type C testing, valve 2CSL"AOV101 was found to be leaking excessively. Efforts to quantify the leakage resulted in a determination that the valve leakage was 856.480 SCFH. This was added to the Type B and C total leakage calculated in procedure N2-ISP-CNT-R003. It was discovered that this leakage resulted in the maximum pathway total exceeding the Appendix J Acceptance Criteria of 0.6 La. Subsequent to this date four valves also exhibited high leakage rates and could not be pressurized to test pressure (Pa). These valves are 2RHS "AOV16A, 2RHS "AOV39B, 2RDS "AOV132 and 2CSH "AOV108. A minimum pathway analysis was performed as each of these valves failed and the results indicated values less than La. The subject valves were repaired and retested.

Prior to restarting the plant from the outage it was determined, in accordance with N2-ISP-CNT-R003, that the "as left" maximum pathway leakage was below 0.6 La.
