1. NRC Ma 7 1986 LETTER COMMENT Generic issues 1, 2, and 3 from SSER-2, Section 3.10.1".3 need to be confirmed.

SS ER-2 SECTION 3. 10. 1. 3 (1) I SSUE STATEMENT The master list of equipment provided by the applicant in preparation for the audit did not fully agree with the qualification and installation status observed during the audit. Since this list is used by the staff to assess the status of the qualification program, the master list should be revised so that it provides the correct status. The applicant must confirm before fuel load that the master list is accurate and up to date.

RESPONSE

The seismiclhydrodynamic qualification master list contains the qualification status of equipment. The master lists for both GE-NSSS and balance-of-plant equipment are enclosed. These documents have been updated to indicate recent status.

Niagara Mohawk has performed a Compliance and Verification review of the Seismic Qualification Program. All required equipment is qualified except for the Invessel Tool Storage Rack. This rack is used for maintenance activities during refueling outages. The invessel Tool Storage Rack will be stored in a location outside of the primary containment and will not be used until it is qualified. All equipment will be installed as qualified prior to declaring that component operable in accordance with the Technical Specifications. Installation is administratively controlled by the Master Tracking System. The Master Tracking System will be used to assure that components are returned to serv.ice.

SSER-2 SECTION 3.10.1.3 (2) ISSUE STATEMENT Upon completion of the seismic and dynamic qualification program, the applicant must confirm that all safety-related equipment is qualified and properly installed.

RESPONSE

All safety-related equipment is qualified as reflected in the master list.

The In-Vessel Tool Storage Rack (MPL F16-E006), is not qualified but is not planned to be used until it is qualified.

Also, refer to Item SSER-2, 3.10.1.3 (1) for further information.

SSER-2. Section 3.10.1.3(3) Issue Statement The nozzle loads and g values assumed for qualification of equipment should be verified and confirmed after the completion of the as-built piping analysis.

SSER-2. Section 3.10.2.3(3) Issue Statement The applicant shall confirm that new loads resulting from loss-of-coolant-accident (LOCA) or analysis of as-built conditions applicable to pump and .

valves important to safety do not exceed those loads originally used to qualify the equipment.

RESPONSE

The as-built piping analysis program has been completed. The nozzle loads and g values determined from these analyses have been evaluated, and it is confirmed that they do not exceed the levels that are used in the qualification of safety related pipe mounted equipment and pumps. The loads considered in the as-built reconcilation include those from loss-of-coolant- accident (LOCA),

SRV discharge. seismic OBE and SSE, and other dynamic events and operating conditions.

2. NRC. MAY 7 1986 LETTER COMMENT A commitment is still needed for item 6 from SSER-2, Section 3.10.1.4 (maintenance and surveillance program).

SSER-2 SECTION 3.10.1.4 (6) question The applicant should commit within the FSAR to establish a maintenance and surveillance program to maintain equipment in a qualified status throughout plant life.

Res onse/Resolution: The applicant responded to this issue by letter dated May 30, 1985. The applicant has not made the FSAR commitment. This issue is confirmatory.

RESPONSE

Section 6.0 of the Equipment Qualification Document describes our program for equipment maintenance and surveillance.

TABLE 3.1 NSSS-3 DESCRIPTION: PERTINENT FINDINGS RHR Heat Exchanger Stresses from the relative horizontal deflections of the primary structure (south reactor auxiliary bay) were not addressed in the dynamic analysis of the RHR heat exchanger.

RESPONSE

A reanalysis of the RHR Heat Exchanger was performed. The linear finite element model of the Heat Exchanger and the support structure was modified to accept the building displacement data. The resulting load cases were added to the existing static and dynamic load cases. The refined Heat Exchanger stresses were only 3.6% above the values reported in the SQRT Qualification Summary and were well within the allowables. The linear finite element model does not model the clearances at the Heat Exchanger-to-support interfaces (which are provided to allow for Heat Exchanger thermal growth but would also reduce loads transmitted from building movements) and therefore is conservative in prediction loads due to building displacements. The stress summary table which follows was revised to show new stress results at the key locations at the upper and lower supports.

TABLE 3.1 BOP-2 DESCRIPTION: PERTINENT FINDINGS Remote Shutdown Panel: The qualification document for the panel structure was presented, but qualification documentation for the devices housed by the structure was not available. The applicant must confirm the qualification of the devices housed in the panel.

RESPONSE

The outstanding device qualification issue for BOP-2 (devices within SWEC Mark No. 2CES*PNL405) have now been completed. The device qualification Test Report TS 6852-10 has been approved by SWEC and may be found in SWEC File No. STRS 07.500-5010E. This report is supplemented by the SWEC report entitled Development of Required Response Spectra from In-situ Modal Test Data for Remote Shutdown Panel 2CES*PNL405, which may be found in SWEC File No. STRS 07.500-5010F.

TABLE 3.1 BOP-3 DESCRIPTION: PERTINENT FINDINGS Foxborro N-2ES Cabinets: The maximum contact chatter duration during testing was not precisely defined and subsequently justified. This finding is generic for all Foxborro devices.

RESPONSE

In Foxborro's generic test program, contact chatter was observed during the SSE phase of the testing for only one type of device a relay. The duration of this chatter is not recorded in the test report and is not available from the vendor. However, no chatter was observed during any of the 20 biaxial OBE tests (2 orientations, in and out of phase), each of 30 seconds duration. The devices functioned normally during these OBE tests. Since the NMP2 SSE response spectra are enveloped by the OBE test response spectra with adequate margin, the devices are considered qualified to the seismic levels of the Nine Mile Point Unit 2 without any anomalies.

a. Foxboro N2ES racks: Provide the acceptance criteria used for monitoring chatter duration, and a justification of this acceptance criteria (if greater than 2 msec) for the various Foxboro devices.
b. For the contact output isolator 2AO-L2C-R which chattered during the SSE tests, no credit can be taken for this test. Therefore, provide a justification that the five generic OBE tests conducted by the vendor were of adequate duration to envelope the NMP2 requirements of six seismic events (i.e. five OBE's and one SSE) ~

RESPONSE

The seismic testing of the Foxboro racks included monitoring contact chatter of the various devices with monitors having minimum resolution of 100 micro-seconds. This is stated on Page 8 of Foxboro Report gOAAA20 Part 1, Rev. A, SNEC File No. IEEE 07.520-5028A. This report also shows seismic test set-ups with contact chatter monitors shown (Pages 47 thru 51). No chatter in excess of 100 microseconds was recorded in any seismic tests, except for the generic SSE test of contact output isolator 2AO-L2C-R. (See the attached letter from the Foxoboro Company.)

b. In the seismic testing of device 2AO-L2C-R, the test duration for each of the five random, multifrequency tests (each in two orientations, in and out of phase) which enveloped Foxboro's generic OBE spectra, was 30 seconds. The fifth generic OBE test may be regarded as meeting the NMP2's one SSE requirement, since the test spectra envelope NMP2's SSE Required Response Spectra with adequate margins (see attached comparison), without any anomaly. The first four generic OBE tests are then compared to the NMP2's requirements for simulating five OBE events. It is noted that IEEE Standard 344-1975 (endorsed by NRC Regulatory Guide 1.100) required a minimum of 15 seconds test duration to allow a reasonable probability of occurrence of the expected excitation for each seismic event. Thus, the total minimum required test duration to age the device for five OBE events is 75 seconds. Based on the actual test duration of 120 seconds for the first four generic OBE's, coupled with the fact that the tests have large margins and therefore produced many more equivalent cycles in the equipment response than those required, it is concluded that the test duration adequately envelopes NMP2's requirement of five OBE's and one SSE.

TABLE 3. 1 BOP-3 DESCRIPTION: PERTINENT FINDINGS Foxborro N-2ES Cabinets: The cabinet was qualified with a 50-ft-lb torque on mounting bolts. The applicant must demonstrate how this torque value will be maintained on regular mounting bolts during the entire life of the plant, given possibility of relaxation and creep.

RESPONSE

The panel, including devices, was qualified by random multi-frequency testing. Connection to the test table was by six half-inch diameter bolts, torqued to 50 ft. lbs.

The field connection utilizes six half-inch diameter weld studs with nuts torqued to 50 ft. lbs. and secured by Loctite adhesive. As previously indicated, this torque value can be expected to generate six kips clamping force in each of the studs. The calculated applied seismic load does not exceed two kips in the most critical bolt.

The dynamic response of the panel at the device mounting locations is not expected to change so long as the connected members retain compression and there is no separation. A zero net compression during an SSE event could occur only if the clamping force drops to about 1.7 kips. This corresponds to a joint preload relaxation of 72/.. Since relaxation is not expected to exceed about 25%, sufficient resultant joint compression wi 11 be available during a seismic event. Therefore, the acceleration levels at the device locations will not exceed the levels achieved during seismic testing and the qualification of the devices will be maintained.

TABLE 3.1 BOP-4 DESCRIPTION: PERTINENT FINDINGS 600 VAC Emergency Panel: Actual installed equipment is mounted differently than the tested equipment; i.e., the installed equipment has a space behind the four mounting points, whereas the tested equipment was flush against the horizontal supports. The applicant must demonstrate similarity between tested and installed conditions.

RESPONSE

The panelboard mounting deviation at the site was determined to be acceptable by performing new seismic tests in which the actual NMP2 installation was duplicated. The test results are documented in an approved test report, BBC Brown Boveri, Inc., Report No. 48-56081-SS, S&N File No. STRS 01.420-5000D.

TABLE 3.1 BOP-5 DESCRIPTION: PERTINENT FINDINGS Diesel Generator: No justification was provided for acceptabilityof the anomalies observed during testing of various devices.

RESPONSES The observed anomalies are addressed in the previously referenced vendor letter dated 8/13/85 (copy attached). As noted in the previous response to the NRC for this item (NMP2L 0646), requirements for periodic surveillance are identified in the EQ maintenance data sheets (copy attached).

Further, permatex has been used for the set screw identified in the anomaly pertaining to control valve PEV-18. The differential pressure gauge will be verified to be tightly secured after each OBE and annually.

This has been included in the preventative maintenance program. For the anomaly observed in the exploratory testing of the main air start control valve, an explanation has been provided by the vendor in the attached August 6, 1986 letter.

TABLE 3.1 BOP-7 DESCRIPTION: PERTINENT FINDINGS Service Water Pump: The motor hold-down bolts do not have the necessary nuts and might become loose from vibration of the motor and seismic loads.

The applicant must confirm the adequacy of hold-down bolts and nuts.

RESPONSE

The Service Water Pump motor hold-down bolts do not use nuts since these fasteners screw into a threaded hole in the supporting bed-plate. These bolts are initially prestressed. The bolt tension corresponding to the pretorque value is 3.5 times higher than the bolt load calculated due to the combined operating'nd seismic conditions, so that joint wi 11 remain tight under design conditions. The motor feet are also dowelled into the bed-plate to prevent misalignment during operation.

In addition, these pumps are monitored for vibration to detect deviations and trends in performance which may occur as a result of bolt loosening (Ref. ISI Program, Section 2, Appendix A). The pumps are also subjected to functional and periodic in-service inspections (in accordance with ASME XI) to ensure maintenance of these design performance criteria.

2) GE Design Record File No. E12-00022-2 During the NliP-2 SQRT audit held at the Nine Nile Point Unit 2 site July 9-12, 1985 the NRC reviewer expressed concern that displacements of the building during a seismic event would cause significant loading in the RHR Heat Exchanger.

GE explained that even though the building displacements were not cons1dered in the structural analysis, their effects would be small. This was based on (1) the short vertical distance between the building support points of the Heat Exchanger supporting structure (about 10') and (2) clearances between the Heat Exchanger and its supports allow for thermal growth which would likely make any loads due to building movements negligible.

In order to confirm the above a r~eanalysis of the RHR Heat Exchanger has been performed. The linear finite element morsel of the Heat Exchanger and the support structure was modified to accept the building displacement data from Reference 1. The resulting load cases were added to the existing static and dynamic load cases. As predicted, the ref1ned Heat Ex'changer stresses were only 3e6% above the values reported 1n the SQRT Qualification Summary and were well within the allowables. The linear finite element model does not conta1n the previously discussed clearances at the Heat Exchanger-to-support 1nterfaces and therefore is conservative in predicting loads due to building displacements.

The attached stress summary table was revised to show results at key locations at, the upper and lower supports, This analysis is documented in Reference 2.

. and Cooper Bessemer, we shall respond to the questions issued fn your telexas regarding the anomolfes reported fn the sub)ect seismic report; Reference enclosure 1, page 4. The, control valve, 2-01Y-044-001 axhfbfted some indeterminate amount of leakage beneath the top Inounted set screw. This control device fs shown functionally on our drawing KSY-36-16 as device number PEV-18. This device serves a shutdown function and therefore fs inactive in the emergency mode of operation.'he leakage occurring at the .<<et screw would have emanated froII connection E, which takes its source from the turbocharger blower discharge air pressure'. The ratio of volume<< of air fn this combustion air system versus the possible leakage free this set screw fs so great that the leakage would not cause any deterioration fn engine performance.

The reasons for this nofse are that the supply and output lines were very small for a 2-1/2" valve as can be Tangy seen fn enclosure 3, photo 3 and photo 4. Additionally, the supply regulator was of relatively low flow capacity. hs a result, the pilot and output pres<<ures both built up slowly during pretest valve operation. This, coupled with a valve open time of 5 seconds, caused the output pressure to only reach approximately 150 psf and therefore caused the pilot pressure to dr op off Nomentarfly when the valve opened. However, no structural faf1ure occurred whfch caused any loss of pr essura. The test rasponse spectrum, as shown on pages 253 to 266, are substantfally hfgher than the riqufred response spectrum. Thts overtestfng no doubt contrfbutad to loosenfng of thfs eeuntfng assembly. It fs therefore concluded that thfs dffferentfal pressure gauge successfully meets the requfrements of sefsmfc testfng.
