NOTICE OF VIOLATION

(NRC INSPECTION REPORT NOS. 50-259/92-11, 50-260/92-11,

AND

50-296/92-1

This refers to the inspection conducted by C. A. Patterson of this office on

March 14, 1992 - April 17, 1992.

The inspection included a review of activities

authorized for your Browns Ferry facility. At the conclusion of the inspection, the

findings were discussed with those members of your staff identified in the

enclosed report.

Areas examined during the inspection are identified in the report.

Within these

areas, the inspection consisted of selective examinations of procedures and

'epresentative

records, interviews with personnel, and observation of activities in

progress.

Based on the results of this inspection, certain of your activities appeared to be in

violation of NRC requirements,

as specified in the enclosed Notice of Violation

concerning ineffective correction action and failure to perform independent

verification. We are concerned about the violation for ineffective corrective action

because

an inspector has identified again that control room recorders contained the

wrong chart paper.

This indicates

a lack of attention to detail.

We are concerned

about the violation concerning independent verification because

equipment

operation was and can be adversely affected.

The examples cited indicated

problems with understanding how and when to perform independent verification.

These problems are particularly of concern since two previous violations involving

independent=verification

were cited within the past two years.

On June 14, 1990,

violation 90-14-05 was issued for failure to perform independent verification during

cable determination.

On August 6, 1991, violation'91-24-02 was issued after the

incorrect fuses were installed in a emergency diesel generator control circuit.

These violations indicate continuing problems with implementation of independent

verification.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

In your response,

you should document the sp'ecific actions taken and any additional actions you

plan to prevent recurrence.

After reviewing your response to this Notice, including

your proposed corrective actions and the results of future inspections, the NRC will

determine whether further NRC enforcement action is necessary to ensure

compliance with NRC regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice", a copy of this

letter and its enclosures

w'ill be placed in the NRC Public Document Room.

The responses

directed by this letter and its enclosures

are not subject to the

clearance procedures of the Office of Management and Budget as required by the

Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
