This refers
to the inspection
conducted
by
C.
A. Patterson
of this office on
July
16,
1991
August
16,
1991.
The
inspection
included
a
review of
activities authorized
for your Browns Ferry facility.
At the conclusion of the
inspection,
the
findings
were
discussed
with
.hose
members
of your staff
identified in the enclosed
report.
Areas
examined
during
the
inspection
are
identified in the report.
Within
these
areas,
the inspection
consisted
of selective
examinations
of procedures
and
representative
records,
interviews with personnel,
and
observation
of
activities in progress.
Based
on the results of thi s inspection,
cer ain of your ac ivities appeared
to
be in violation of
NRC requirements,
as specified
in the enclosed
Notice of
Violation concerni ng removal
o
fire wrap and fuel movements.
We are concerned
about the first violation because fire wrap that separates
redundant trains of
safe
shutdown
equipment
was
removed
from operable
equipment without posting
a
The fire wrap was
removed to support Unit 3 walkdown inspections
which were
planned
by contractor
personnel.
These
inspections
had previously
been
performed
and should not have
been permitted.
We are concerned
about the
second violation because
two fuel
movement errors
occurred within
a
two week
period.
Corrective
actions
were
implemented
after
the first
occurrence
including independent verification but
a second
error still occurred.
Both of
these
violations
involved
Unit
3 activities
and
control
of
contractor
personnel.
You are
required
to respond to this letter and should follow the instructions
specified
in the
enclosed
Notice
when
preparing
your
response.
In your
response,
you should
document
the specific actions
taken
and
any additional
actions
you plan to prevent recurrence.
After reviewing your response
to this
Notice,
including your proposed
corrective actions
and
the results of future
inspections,
the
NRC will determine
whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regula.ory requirements.
