ENCLOSURE 1

NOTICE OF VIOLATION Tennessee Valley Authority Browns Ferry 1, 2, and 3

Docket Nos. 50-259, 50-260, and 50-296 License Nos.

DPR-33, DPR-52, and DPR-68 During the Nuclear Regulatory Commission (NRC) inspection conducted on August 18 - September 15,

1990, a violation of NRC requirements was identified.

The violation involved the bypassing of gC holdpoints during maintenance and modifications work activities.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions, "10 CFR Part 2, Appendix C

(1990), the violation is listed below:

Technical Specification (TS) Section 6.8.1, Procedures, requires that written procedures shall be established, implemented and maintained covering applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.

Appendix A of Regulatory Guide 1.33 includes administrative procedures and procedures for controlling repair activities of safety-related equipment.

Site Directors Standard Practice 3.11, guality Control Inspection

Program, Section 3.1.2.e.

1 requires that during work instruction preparation inspection planning will include holdpoints and notification points sufficient to provide opportunity for witness/inspection of specified activities.

SDSP-3. 11, Attachment A requires that the work activity shall not progress past the guality Control inspection holdpoint until signed as accepted by an inspector or until the Site guality Nanagement or his designee's consent to continue is obtained and documented.

Contrary to the

above, between January 18,
1990, and September 1,
1990, the requirements were not met in that fourteen examples of bypassing documented gC holdpoints were identified by the licensee'ite guality Organization.

Five examples occurred in August 1990.

This is a Severity Level IV Violation (Supplement I) applicable to all three units.

Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit a written statement or explanation to the Nuclear Regulatory Comission, ATTN:

Document Control

Desk, Washington, DC 20555, with a

copy=- to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, Browns Ferry, within 30 days of the date of the letter transmitting this Notice.

This reply should be clearly marked as a "Reply to a Notice of Violation" and should include:

achieved, (4) the corrective steps which will be taken to avoid-further violations, and (5) the date when full compliance will. be achieved..

Where good cause is shown, consideration will be given to extending the response time.
