This refers to the Nuclear Regulatory

Commission

(NRC) inspection

conducted

by

D.

R. Carpenter

on November

15 - December

18,

1989.

The inspection

included

a

review of activities

authorized

for

your

Browns, Ferry facility.

At the

conclusion of the

inspection.,

the findings were discussed

with those

members

of your staff identified in the enclosed

inspection report.

Areas

examined

during

the

inspection

included

surveillance

observations,

licensed

power

indication,

maintenance

observation,

operational

safety

verification, restart

test

program,

and site

management

and

organization.

Within these

areas,

the

inspection

consisted

of selective

examinations

of

procedures

and

representative

records,

interviews

with

personnel,

and

observation

of activities in progress.

The

inspection

findings indicate certain activities

appeared

to violate

NRC

requirements

and other activities appeared

to deviate

from one of your previous

commitments. 'he

violations

involved the failure to

respond

in

a timely

manner

to off-normal conditions

and

to initiate Condition Adverse

to guality

Reports

for disposition

of significant

hardware

test

exceptions.

The

deviation

concerned

your failure to submit,

a special

report in accordance

with

a

licensee

commitment.

The

violations

and

deviation,

with references

to

pertinen't

requirements,

an'd

elements

to

be

included

in your

response

are

described

in the enclosed

Notices.

The first violation described

in the enclosed

Notice is similar to

a violation

contained

in the Notice

sent

to

you

by our letter

dated

August

31,

1989.

Because

"Similar Violations,"

as

described

in the

NRC Enforcement Policy, are

of significant concern

to the

NRC,

please

give particular attention

in your

response

to

the identification of the

root cause

of this

problem

and your

corrective action to prevent recurrence.
