ENCLOSURE NOTICE OF VIOLATION Tennessee Valley Authority Browns Ferry Docket Nos.'0-259, 50-260, 50-296 License Nos.
DPR-33, DPR-52, DPR-68 During the Nuclear Regulatory Commission (NRC) inspection conducted on July 21-25, and July 1-31,
- 1986, a violation of NRC requirements was identified.
The violation involved a failure to maintain high radiation area doors locked or guarded.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"
10 CFR Part 2; Appendix C (1986),
the violation is listed below:
Technical Specification 6.3.A requires that detailed written procedures covering testing requirements and radiation control procedures shall be adhered to.
Special Test Procedure No.
86-18, Reactor Mater Cleanup Pump Decontamination, July 25,
- 1986, Step 3.2 required that continuous high radiation area door surveillance was required whenever the Reactor Water Cleanup (RWCU) pump room door was opened during chemical'econtamination activities.
Radiation Control Instruction (RCI)-17, High Radiation Area Door Control, October 16,
- 1985, Paragraph 6.5, required that the individual who requests from the shift engineer for the door to bewnlocked will be responsible for door control while the door is unlocked.
The responsible individual will inform the shift engineer when the door is returned to the locked position.
Contrary to the
- above, the licensee failed to adhere to radiation control and testing procedures in that:
1.
On March 28, 1986, the licensee failed to lock or maintain continuous surveillance (door control) on the high radiation area door to the RWCU heat exchanger room.
2.
On July 27,
- 1986, the licensee failed to maintain continuous high radiation area door surveillance on the RWCU pump room door that had been opened for the chemical decontamination work.
