Dear Mr. Grier:

This serves as an update to our interim report regarding the failure of valve manufacturers to stake the stem lock riuts'astening Limitorque Mode'1 No.

SMB and SMC motor operators to valves.

The 'deficiency was initially reported under the provisions of 10 CFR 50.55(e) by PPGL letter PLA-329, dated March 2, 1979, which advised that the Commission would be further advised of PPGL's corrective action and a date for completion thereof.

To date it has been determined that this deficiency pertains only to rising stem type valves and a total of 658 valves have been identified within the Bechtel scope of supply as being suspect with regard to unstaked stem lock nuts.

In addition, Bechtel has been requested to identify, inspect and correct the valves within the G.E. scope of supply which are similarly affected.

Field personnel are in the process of inspecting all valves which are presently suspect and Bechtel construction forces are staking all nonconforming lock nuts.

Approximately 40% of the valves inspected so far have required corrective action.

The inspection and correction effort is continuing and is expected to be completed in the spring of 1980. It is anticipated that the final report on this deficiency will be issued by April 15, 1980.
