ENCLOSURE 1

NOTICE OF VIOLATION Tennessee Valley Authority Browns Ferry 1, 2, and 3

Docket Nos.

50-259, 50-260 and 50-296 License Nos.

DPR-33, DPR-52 and DPR-68 The following violation was identified during an inspection conducted on October 26 - November 20, 1985.

The Severity Level was assigned in accordance with the NRC Enforcement Policy ( 10 CFR Part 2, Appendix C).

4. 1.A requires that instrumentation systems shall be functionally tested and calibrated as indicated in Tables
4. 1.A and
4. 1.B respectively.

Instrument Calibration, requires that the Main Steam Line High Radiation Instrument Channel be calibrated using a radiation source each refueling outage.

Contrary to the

above, the Main Steam Line High Radiation Instruments were not calibrated using a radiation source during the End-of-Cycle (EOC) 5 refueling of Unit 1 (April 16, 1983 to January 2,

1984),

the EOC-4 refueling outage of Unit 2 (July 30, 1982 to March 18, 1983),

and the EOC-5 refueling outage of Unit 3 (September 6,

1983 to December 1, 1984).

Due to a deficiency in the plant instruction used to comply with the Technical Specification (Surveillance Instruction, SI 4. 1.B-1&, Main Steam Line Radiation Monitoring System Calibration), the requirements of an instrument calibration were not satisfied.

Technical Specification 1.V. 1 defines an instrument calibration as an adjustment of an instrument signal output so that it corresponds within an acceptable range and accuracy to a

known value of the parameter which the instrument monitors.

Although SI.4. 1.B-10 was performed on these instruments and it requi red the use of a radiation source, it did not contain an acceptable range and accuracy and no adjustment of the signal output was required.

No evaluation was performed to determine the accept-ability of instrument output signals ranging from 25 mrem/hour to 200 mrem/hour for the same radiation source strength input to the radiation monitors.

This is a Severity Level V violation (Supplement I) and is applicable to all three units.

Pursuant to 10 CFR 2.201, you are required to submit to this office within 30 days of the dat'~of this Notice, a written statement or explanation in reply, including:

(1) admission or denial of the alleged violations; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.

Security or safeguards information should be submitted as an enclosure to facilitate withholding it from public disclosure as required by 10 CFR 2.790(d) or 10 CFR 73.21.
