Dear Mr. Grier:

This letter serves to confirm information provided by telephone to NRC Region I representative Mr. R. McGaughy by Mr. A. R. Sabol of PPaL on August 21, 1980.

During that conversation, Mr. McGaughy was advised that the subject condition was under evaluation for reportability under the provisions of 10CFR50.55(e).

The deficiency involves limit switches on Limitorcpxe valve operators that control the valve operator and are used to provide position indication.

During preoperational checkout of the Limitorcpxe valve operators, cracks were found in the limit switch rotor, The rotor is made of a phenolic material and is staked to a metal pinion shaft. As the valve operates, the limit switch rotor turns, rotating a moving contact which opens or closes a limit switch contact. The cracks were found in the limit switch rotor where the pinion shaft. The rotor cracks appear to be caused by a misalignment in it is staked to the holes in the limit switch rotor and the pinion shaft.

Since the limit switches are used for the control of the valve operator and foz indication for valve position, a defective limit switch rotor could result in the failure of a valve to operate and/or provide incorrect indication of the valve position. In that these types of failures cculd have an'adverse effect on the safe operation of the nuclear power plant, the defect is considered as reportable under the provisions of 10CFR50.55(e) . A letter was sent to Limitorcpe Corporation which identified the problem and recpested them to determine the cause, scope and corrective action for the limit switch rotor problem.

Bechtel NCR 6020 and PPK NCR 80-134 have been written to document known rotor defects in the Core Spray and Reactor .Recirculation systems. Upon engineering evaluation of the data that Limitoraue provides, an inspection program will be developed and implemented for the conduct of field inspections of the limit switch rotors. All defective rotors will,be replaced.
