This is in response to R. C. Lewis'pril 9, 1981 letter to H. G. Parrisf Report Nos. 50-259/81-09,

-260/81-09, and -296/81-09 concerning activities at the Browns Ferry Nuclear Plant which appear to have violated NRC requirements.

50-259/81-09, 50-260/81-09, AND 50-296/81-09 R. F. SULLIVAN'S INSPKTICN CN FEBRIIAHY 26 TO MARCH 25, 1981 ITEM A Technical Specification 6.3.A.5 requires detailed written prccedures shall be prepared and adhered to for preventive or corrective maintenance operations which could have an effect on the safety of the reactor.

Browns Ferry Standard Practice 6.1, Performance of Maintenance, requires that routine maintenance will be documented by use of a trouble report.

Contrary to the above, the docmentation of maintenance by use of a trouble report was not met in that on February 20 and 21, 1981, maintenance was performed on the scram discharge volume continuous monitoring system without documentation of the work performed.

This is a Severity Level V Violation (Supplement I.E.) applicable to unit l.

1 Admission or Denial of the All ed Violation TVA admits the violation occurred as stated.

Reasons for the Violation if Admitted On February 20 and 21, 1981, defective cables were replaced on the scram discharge volume continuous ultrasonic monitoring system without proper documentation of work performed.

The Level II ultrasonic technician performed this maintenance which was within the scope of his qualification. It was felt at the time that this work fell into the category of adjustments to the ultrasonic test system.

Corrective Steps Which Have Been Taken and Results Achieved Trouble reports 158111 and 180857 were generated to document work performed.

Corrective Steps Which Will Be Taken to Avoid Further Violations Date When Full Personnel involved in handling the ultrasonic test system have been made aware that all maintenance must be documented in accordance with plant instructions.

$1 Technical Specification 6.3.D.2 requires that each high radiation area in which the intensity of radiation is greater than 1000 mrem/hr shall be locked, and the keys maintained under administrative control of the shift engineers on duty.

Contrary to the above, on March 24, 1981, the keys to these 'areas were being maintained and used by health physics personnel and radwaste operators without being under the administrative control of the shift engineer on duty.

This is a Severity Level V Violation (Supplement I.E.).

Reasons for the Violation if Admitted At the time of the occurrence, no formal written procedure existed which specifically stated how the shift engineer on duty was to exert administrative controls over the high radiation area keys.

t Corrective Ste s Which Have Been Taken and Results Achieved On March 27, 1981, operations instruction letter number 13 was revised implementing strict controls over the high radiation area keys.

This SIL requires that at least twice per shift the shift engineer on duty is informed as to the status of each high radiation area key.

High radiation area grand master keys have been assigned to the health physics and public safety sections.

These sections report to the shift engineer each shift as to the status of these keys.

Furthermore, the usage of these keys is limited to only these sections.

Usage of all other high radiation area keys is limited to the operations section only.

Only operators may check out these keys and each key checked out must be turned in before shift turnover.

All other keys have been confiscated, except for three, which have been identified as having been lost.

To avoid confusion, all high radiation area keys have been distinctively marked.
