Identification and Descri tion of Occurrence:

During startup following a re ueling outage, IRM's B, D, and H did not respond.

Conditions Prior to Occurrence:

Unit 1 at 99X power.

Unit 2 startup following refueling outage Unit 3 at 87% power Action specified in the Technical Specification Surveillance Requirements met due to ino erable e ui ment.

Describe.

All rods inserted within the required 4-hour limit of Tech Spec Table 3.1.A.

A arent Cause of Occurrence:

Open cables in the drywell 'on IRM's B, D, and H.

Anal sis of Occurrence:

I There was no damage to plant equipment.

There was no activity release, no personnel exposure or injury and no effect on the health and safety of the public.

Corrective Action

Repaired cables and connectors.

SMI 92.7 has'een written to identify any problems at cable reroute.
