CAUSE DESCRIPTION AND CORRECTIVE ACTIONS Q27

Cause of event

was failure to request technical specification revision to incorporate the exposure extended MAPLHGR limi'ts for up to 40,000 Mwd/t.

New valves were entered into computer and a proposed T.S. revision submitted.

MAPLHGR values obtained L

after exceeding 30,000 Mwd/t were found to be conservative compared to new limits.

Action specified in the Technical Specification Surveillance Requirements met NA

~A~arent Cau.e of Occurrence:

Failure to modify process computer software to incorporate the exposure extended MAPLHGR limits for up to 40,000 1M/t.

Analysis of Occurrence:

There was no damage to plant equipment.

There was no activity release, no personnel exposure or injury and no danger to the health or safety of the public.

Corrective Action

The process computer software was modified to incorporate the extended MAPLHGR limits up to 40,000 1Md/t.

The process computer output was inspected to verify the MAPLHGR values checked daily were within limits.

Proposed Tech.
