APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority Browns Ferry License No. DPR-33 Based on the NRC inspection November 6-9, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below.
These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by Technical Specification 6.2.B.4.C, the Plant Operations Review Committee (PORC) reviews proposed changes to equipment or systems having safety significance.
Contrary to the above, the PORC had not, as of November 9,
- 1979, reviewed changes prior to their installation, to Unit 1 safety related equipment as described in the following Design Chnage Requests (DCRs):
1.
Replacement of the refrigerant purifier system on control bay chiller 1B with a large permanent reclaimer (DCR P-1840).
2.
Addition of a hand valve and a relief valve to the control bay chiller oil cooler (DCR P-1858).
3.
Replacement of a liquid indicator on control bay chiller oil separator assembly piping with a check valve (DCR P-1889).
This is an Infraction.
