The p&ion gear was installed: backward in the valve operator.

This allowed the gear to move on the motor shaft and disengage its mating gear such that the valve would not operate.,

Anal sis of Occurrence:

No ganger to e

tE or safety of the public, no significant resulting occurrence or chain of events.

Corrective Action

Valve operator was replaced, MMX-88 implemented to have second person verification and inspect'ion.
