APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority Browns Ferry License No. DPR-33 DPR-52 DPR-68 Based on the NRC inspection March 12-15, 1979,'ertain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below.

These items have been categorized as described in corres-pondence to you dated December 31, 1974.

As required by 10 CFR 50, Appendix B, Criterion XVIII, and implemented by the licensee equality Assurance Program Description (Topical Report-TVA-TR75-1A),

a comprehensive system of planned and periodic audits shall be carried out to verify compliance with all aspects of the quality assurance (gA) program.

Provisons of ANSI N45.2.12 (Draft 3, Rev. 4-February 1974) and gA Procedure OP-JAP-18.1, as adopted by the Topical

Report, require a written response to adverse audit findings and the inclusion of proposed corrective actions and completion dates when required.

(}A Procedure OP-gAP-18.1 also requires that audit findings be given noncompliance category designations.

Contrary to the above, the following are examples of failure to meet the requirements of ANSI N45.2.12 and of gA procedures for the period January 1978 through January 1979:

1.

Adverse audit findings were not responded to for audits OPIA-78-2, 78-3, 78-5, 78-6, 78-8 through 12, 78-SP-7, 78-SP-10, 78-SP-12 and 78-SP-13.

2.

Written responses from the audited organization for audits OPIA-78-1, 78-6, 78-11 and 78-SP-3 did not contain proposed corrective actions and completion dates.

3.

Audit findings were not properly designated as to noncompliance category for audits OP(AA-78-10 through 13 and 79-1.

This is an infraction.
