While preparing to refuel, secondary containment integrity was not conclusively demonstrated prior to breaking primary containment.

Conditions Prior to Occurrence:

Refueling outage.

Action specified in the Technical Specification Surveillance Requirements met due to ino erable e ui ment.

Describe.

Fuel movement not permitted.

A arent Cause of Occurrence:

The test to demonstrate secondary integrity was improperly conducted due to a combination of personnel error and procedural inadequacy.

The surveillance instruction, required to prove that the s'tandby gas treatment system could maintain one-quarter inch of water vacuum with a system in. leakage of not more than 3.2,000 cfm, contained incorrect data for the. condition specified.

Anal sis of Occurrence:

Theqe was no hazard to the public health or safety.,

Corrective Action

When the error in the data was discovered, fuel handling was immediately stopped and adequate secondary containment was verified.

The surveillance test is being revised for clarity.

A. two-party check is being added to the master refueling test instruction.
