NRC INTEGRATED INSPECTION REPORT 50-400/96-11
Dear
Hr . Robinson:
On January 4,
1997, the
NRC completed
an inspection at your Harris reactor
facility.
The enclosed report presents
the results of that inspection.
During the six weeks covered
by this inspection period, our inspectors
found
thai your staff generally took
a safety conscious
approach to the activities
conducted at the Harris Plant.
We believe that your decision to remove the
unit from service to repair
an unisolable
steam leak on
a small gland seal
line reflected the appropriate safety attitude.
Three violations were
identified during the period.
The first, which involved two examples of
failure to follow procedures
related to control
room chart recorders
and
temperature
monitoring for the boric acid heat trace system, is of concern
because
your operations staff is currently attempting to correct
a negative
performance trend.
The second,
which involved a missed surveillance
on
auxiliary control panel
equipment
due to a procedure
inadequacy,
is of concern
because
the procedure
development
and review process
should not allow errors
to be inadvertently introduced.
Our concern
on the third violation, which
involved corrective action problems related to the emergency service water
structure air handlers,
is that it represented
an insensitivity to the use of
appropriate
approved
problem resolution methods.
These violations are cited in the enclosed
Notice of Violation, and the
circumstances
surrounding the violations are described in detail in the
enclosed
report.
Please
note that you are required to respond to this letter
and should follow the instructions specified in the enclosed
Notice when
preparing your response.
The
NRC will use your
response,
in part, to
determine
whether
further enforcement action is necessary
to ensure
compliance
with regulatory requirements.
