NPF-63 During an NRC inspection conducted November 20 - December 17,

1993, a

deviation from a written commitment was identified.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C, (1993),

the deviation is listed below:

I NRC Inspection Report 50-400/93-12 contained a Notice of Violation (400/93-12-02) documenting the failure of plant personnel to verify

that, when racking in 6.9 KV circuit breaker 122, the mechanism operating cell align with the corresponding fork on the breaker mechanism.

In= a letter dated July 30, 1993, the licensee

.responded to the violation by stating that several corrective actions had already been completed and that a final action, the installation of placards inside the 6.9 KV breaker cubicles to indicate the location for conducting mechanisms operating cell alignment verification, would be complete by October 1,

1993.

Contrary to the above, as of December 17, 1993, the placards had not been placed in the affected 6.9 KV circuit breakers cubicles and no request for an extension to the implementation date had been submitted to the NRC.

Please provide to the U.S. Nuclear Regulatory Commission, ATTN:

Document Control Desk, Washington, D.C. 20555, with a copy to the Regional Administrator, Region II, and if applicable, a copy to the NRC Resident Inspector, in writing within 30 days of the date of this Notice, the reasons for the deviation, the corrective steps which have been taken and the results

achieved, the corrective steps which will be taken to avoid further deviations, and the date when your corrective action will be completed.

Where good cause is shown, consideration will be given to extending the response time.
