Attachment to NRC-804 REPLY TO A NOTICE OF VIOLATION NRC INSPECTION REPORT NO. 50-400/92-30 VIOLATION 400 92-30-01 Re orted Violation Technical Specification 6.8.1.a requires that written procedures be established and implemented covering pro,.edures outlined in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.

Regulatory Guide 1.33, Appendix A, paragraphs'.c and 8, require procedures for equipment control.

'dministrative procedure AP-003, General Plant Personnel Safety and Housekeeping, paragraph 5.7, and AP-020, Clearance Procedure, paragraph 5.1.6, requires that electrical equipment shall be de-energized and an equipment clearance established prior to working on electrical equipment.

Contrary to the above, on January 8,

1993, work was performed on the 480 volt power supply circuit to change the direction of rotation for fan AH-26A without prior establishment of an equipment clearance to de-energize the electrical circuit.

This is a Severity Level IV violation (Supplement I).

Denial or Admission and Reason for the Violation:

The violation is admitted.

The violation occurred during performance of post-maintenance testing following the replacement of the motor for fan AH-26A.

Upon initial fan start (bump'or rotation) there was some confusion as to whether the fan rotated in the correct direction.

Operations secured the fan by turning off the control switch.

While the situation was 'eing discussed among licensee personnel, an electrician swapped two motor leads in the fan's power supply junction box to obtain the proper direction of rotation.

No equipment clearance was obtained for the removal of the electrical motor leads.

The electrician involved admitted that the violation occurred due to inattention to detail.

The individual committing this clearance violation has never had a similar violation in approximately eleven years of service.

lt Corrective Ste s Taken and Results Achieved:

The

. individual committing the clearance violation was counseled on the requirements of site procedure AP-020, Clearance Procedure, and specifically the requirements that were not followed.

Disciplinary action was deemed necessary due to the nature and severity of the violation.

Corrective Ste s Taken to Avoid Further Violations:

This violation will be covered in second quarter continuing training for I&C technicians, electricians, and mechanics.

Additional training on AP-020, Clearance Procedure, will also be given at this time.

Date When Full Com liance Will Be Achieved:

Full compliance will be achieved by June 30,

1993, upon completion of the above mentioned training.
