Attachment to NRC-791 REPLY TO A NOTICE OF VIOLATION NRC INSPECTION REPORT NO. 50-400/92-17 VIOLATION 400 92-17-02 Re orted Violation:

10 CFR Part 50, Appendix B, Criterion XVI, requires measures be established to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, and deviations, are promptly identified and corrected.

Contrary to the above, conditions adverse to quality were not promptly corrected.

Improper diesel generator starting air quality was not corrected following issuance of Generic Letter 88-14 and initiation of Plant Change Request PCR 3995, dated November 14, 1988.

This problem contributed to an emergency diesel generator start failure on September 14, 1992.

This is a Severity Level IV violation (supplement I).

Denial or Admission The violation is admitted.

Reason for the Violation While performing test procedure OST-1823, lA-SA Emergency Diesel Generator (EDG) 18-month Operability Test Mode 5 and 6, the 'A'DG frequency failed to stabilize between 58.8 and 61.2 Hz in less than 10 seconds as required by acceptance criteria specified in OST-1823 and Technical Specification 4.8.1.1.2.f.6.b.

The shuttle valve supplying starting air pressure to the governor oil booster for opening the fuel racks was found to have failed.

Water and debris were found in the air lines.

The debris in the air lines is attributed to corrosion and wear products resulting from the inability of the air drying/moisture removal system to maintain the starting air dew point within acceptable limits.

To improve the moisture removal ability of the Starting Air System, PCR-3995 was initiated in November of 1988 to add an aftercooler and a moisture separator between the starting air compressors and the air dryers.

Due to a failure in properly prioritizing PCR-3995, the planned improvements were not implemented and the EDG test failure occurred on September 14, 1992.

Corrective Ste s Taken and Results Achieved:

The shuttle valve was subsequently replaced and the air lines were blown down with clean dry air.

OST-1823 was successfully performed on September 16, 1992.

The 'A'DG was capable of performing its.intended safety function up to the time of the test failure as demonstrated by satisfactory completion of previous surveillance testing.

At, the time of the test failure, the plant was in a mode in which only one diesel was required to be operable.

The 'B'DG was operable at all times in which the 'A'DG was inoperable.

Corrective Ste s Taken to Avoid Further Violations:

As an interim measure, PCR;6584 will be implemented on both the 'A'nd 'B'DG during the current Refueling Outage C4 to provide additional air system filtration to critical diesel components..

Both the 'A'nd 'B'DG air lines will have been blown down with clean dry,. air prior to installation of these

filters, PCR-3995 will be implemented on both the 'A'nd 'B'DG by March 31, 1993, to return the starting air dew point to within desirable limits.

To ensure that adverse conditions identified by a

PCR receive proper prioritization and management attention, the PCR process willbe removed from the list of approved Corrective Action Program (CAP)

Subprograms and an Adverse Condition Report (ACR) will be required for PCRs which document adverse conditions.

This program change will be completed by November 30, 1992.

A review of existing PCRs, not yet implemented, willbe conducted to ensure that an ACR exists for those which identify adverse conditions.

This review will be completed by November 23, 1993.
