The NRC has not made an attempt to ascertain the accuracy of the information supplied by the individual.
The enclosed information is provided for your review so 'that you can take the appropriate action to resolve the matter.
The enclosed information is not being placed in the Public Document Room.
Appropriate safeguards must be applied to the enclosed information in view of its unsubstantiated.
