OPERABILITY OF THE ALTERNATE MINI-FLOW LINES FOR THE CHARGING/SAFETY INJECTION PUMPS AT SHEARON HARRIS NUCLEAR POWER PLANT (TAC No. H84220)

During the week of August 8,

1992, an NRC inspection team was on site to review the effects and circumstances pertaining to a reported loss of high head injection at the Shearon Harris Nuclear Power Plant (SHNPP).

The event was reported to the NRC in Licensee Event Report (LER)91-008 in Hay of 1991, and was assigned a conditional core damage frequency of 6.3xlOE-3 in a preliminary Oak Ridge accident sequence precursor study of 1991 events performed for the NRC.

The inspection centered around the high head alternate minimum flow (AMF) system piping and valves.

This AHF system was installed after a

1980 Westinghouse 10 CFR Part 21 report.

At the conclusion of the inspection, the findings were discussed with your staff during the team exit meeting on August 7, 1992.

The details of team findings and inspection activities will be documented in an inspection report to be issued shortly.

Based on the team findings, the staff has concluded that the corrective actions as described in your LER 91-008 were incomplete and did not address the following potential issues that could affect the operability of the high head injection system:

1.

The lack of analysis for system piping integrity as a result of at least four water hammer events:

a.

1986 water hammer - no corrective action b.

1987 inadvertent safety injection no corrective action c.

1990 weld leakage no corrective action d.

1991 event (LER 91-008) incomplete corrective action with an inadequate root-cause analysis.

2.

The potential for water hammer in the downstream portion of the AMF piping between the AHF safety relief valves (SRVs) and the refueling water storage tank.

3.

The potential for valve chatter and setpoint drift for the SRVs.

August 14, 1992 Based on the team findings, the staff requests that you take the necessary action to resolve the above potential safety issues.

In addition to the above operability issues, the staff requests that you take the actions necessary to ensure that operators can detect an inadequate flow condition during high head injection phase.

Actions taken should include procedural changes to enable operators to cope with this condition.

Because of the high safety significance of these

issues, the staff requests that you meet with the staff during the week of August 17, 1992, to provide your justifications for the continued operability of the high head injection system.

Please be prepared to describe your plan and schedule for corrective actions to resolve the above identified issues.
