NOTICE OF VIOLATION

(NRC

INSPECTION

REPORT

NO. 50-400/91-24)

This refers

to the

inspection

conducted

by J.

Tedrow of this office on

October

19 - November

15,

1991.

The

inspection

included

a

review of

activities

authorized for your Harris facility.

At the conclusion of the

inspection,

the findings

were

discussed

with those

members

of your staff

identified in the enclosed

inspection report.

Areas

examined

during the inspection

are identified in the report.

Within

these

areas,

the inspection

consisted

of selective

examinations

of procedures

and

representative

records,

interviews with personnel,

and observation

of

activities in progress.

Based

on the results

of this inspection,

certain of your activities

appeared

to be in violation of NRC requirements,

as specified in the enclosed

Notice of

Violation (Notice).

We are

concerned

about

the violations

because

they

involved

a failure to provide

an adequate

surveillance test procedure, failure

to

perform

adequate

boric acid

pump testing,

and failure to adequately

identify, document,

and correct deficiencies.

These violations, in addition

to identified weaknesses

regarding failure to recognize that additional

pump

testing

may

have

been

required following maintenance,

failure to maintain

system

design control, failure to implement manufacturer's

recommendations

to

calibrate

heat

trace

control circuits,

and the failure to identify control

circuits related to meeting technical

specification surveillance

requirements,

indicate potential

program deficiencies.

You are required to respond

to this letter and should follow the instructions

specified

in the

enclosed

Notice

when

preparing

your response.

In your

response,

you should

document

the specific actions

taken

and

any additional

actions

you plan to prevent recurrence.

After reviewing your response

to this

Notice, including your proposed

corrective actions

and the results of future

inspections,

the

NRC will determine

whether

further

NRC enforcement

action is

necessary

to ensure

compliance with NRC regulatory requirements.
