ENCLOSURE I NOTICE OF VIOLATION Carolina Power and Light Company Shearon Harris Docket No. 50-400 License No. NPF-53 During the Nuclear Regulatory Commission (NRC) inspection conducted 'n January 5-9, 1987, a violation of NRC requirements was identified.

The violation involved the failure to implement an approved procedure in controlling clearance tagging and equipment status'.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C ( 1986), the violation is listed below:

Technical Specification '6.8.1 requires that procedures be implemented in accordance with Regulatory Guide 1.33, Rev. 2,

1978, Appendix "A".

Appendix "A", Item 1.C recommends procedures for equipment control (i.e.,

locking and. tagging) and Section 9.e.'recommends procedures for control of work and clearances.

Shearon Harris procedure AP-20 requires control'f the processing of clearances including review and

approval, placement and removal of tags, documentation of actions, and verification of proper system alignment following completion of work or cancellation of the clearance.

Contrary to the above, on January 8,

1987, the steam dump valves failed to oper'ate on demand from the control room.

The licensee subsequently determined that a control circuit breaker was unknowingly left tagged out and could produce no supporting documentation.

The licensee believes the clearance was cancelled, but failed to follow the procedure in documenting the disposition, removing the tag, and restoring the equipment to operation.

As a result, the steam dump valves remained inoperable from November 1986 to January. 1987.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Carolina Power and Light Company is hereby required to submit to this.Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including (I) admission or denial of the violation, (2) the reason for the violation if

admitted, (3) the corrective steps which

- have been taken and the results

achieved, (4) the corrective steps which will be taken to avoid further viola-
tions, and (5) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending the response time.
