Independent
Inspection Effort
The inspection effort was directed
at requirements
for physical spatial
separation
for electric
cables
of safety-related
circuits
as specified in
the Final Safety Analysis Report.
The scope of this inspection
was limited
to separation
criteria designed
to protect safety-related
circuits against
damage
from cable fires caused
by uncleared electrical faults.
Electrical
faults are
an anticipated
operational
occurrence;
and, if circuit breakers
fail to clear the fault (single failure criteria),
a cable fire may result.
The as-built electrical installation at Harris I does
not meet the electri-
cal
separation
criteria,
and this
was identified in Inspection
Report
50-400/86-62
as Unresolved
Item 50-400/86-62-01.
The
NRC inspectors
focused their attention
on the following specific areas:
(a)
The
licensee's
Engineering
Department
efforts to resolve critical
situations
involving separation criteria.
(b)
The guality Assurance
Department's
procedures
to inspect the electrical
works for proper cable separation.
(c)
The
NRC inspectors
talked with electrical
engineers
in the Engineering
Department
as well as supervisors
in the guality Assurance
Department
who are
intimately involved with ongoing efforts to achieve
proper
electrical separation.
(d)
The
NRC inspectors
observed
teams of licensee
employees
who were in the
process
of
conducting
inspections
of the electrical
separation
attribute.
A special
inspection
team consisting of Engineering
and
gA personnel will
conduct inspections
to identify electrical
separation
deficiencies,
evaluate
in'spection
findings,
and direct construction
personnel
when modifications
are required to achieve
proper separation.
The guality Assur ance surveil-
lance
group will perform the "final" inspection
walkdown of plant areas
prior to turnover to operations.
This group will document
any deficiencies
found.
In
a
management
level
meeting
at
the
Region II office
on
September
30,
1986,
the licensee
reiterated
his commitment to implement the
above
program for finding and correcting electrical
separation
deficiencies.
The licensee
stated
that
any deficiencies
involving cable of Engineering
Safety
Features
Systems will be corrected
before fuel load and deficiencies
involving all other safety-related
cables will be corrected
before exceeding
5X power level.

As
a result of information obtained
during this inspection,
NRC inspectors
reached
two conclusions.. First,
due to the licensee's
present effort and
recently
developed
programs,
acceptable
electrical
separation
will
be
achieved
in the final installation;
second,
on-site engineering,
construc-
tion supervision
and
the quality assurance
groups substantially failed to
meet their
FSAR commitments with respect
to physical spatial
separation
of
electric
cables
of safety-related
circuits.
Corrective actions
were not
implemented
by the licensee
until
such violations were repeatedly
pointed
out by
NRC inspectors.
Therefore,
Unresolved
Item 50-400/86-62-01 is hereby
upgraded
to
a violation.
This
item will be identified
as
Violation
50-400/86-66-01,
Failure of the guality Assurance
Program to Identify All
Electrical Separation
Deficiencies.
