TITLE:
Reactor Vessel Head Adaptor Tube Cracking BASIC COMPONENT SUPPLIED BY>>
Westinghouse Electric Corporation NATURE OF DEVIATION:
Westinghouse's initial evaluation of information provided by Electricite de France (EdF) determined that a
crack and subsequent leak developed in a reactor vessel head adaptor tube.
These head adaptor tubes are manufactured from Alloy 600 material in both Framatome and Westinghouse designed plants.

DISCOVERY DATE:
January 24, 1992 CORRECTIVE ACTIONS TO DATE:
The Westinghouse Owners Group (WOG) approved a
Project Authorization on February 5, 1992, for Reactor Vessel Closure Head Penetration Alloy 600 Primary Water Stress Corrosion Cracking.
The purpose of the program is to establish the extent of applicability to Westinghouse supplied vessels.
The program started in March,
- 1992, and is applicable to all WOG domestic utilities, expect Yankee Rowe.
