NRC INSPECTION REPORT

NOS. 50"250/89-25

AND 50-251/89-25

This refers to the Nuclear Regulatory

Commission

(NRC) observation of crew

training and assessment

on the Turkey Point simulator, specified in

Confirmation of Action Letter (CAL), 89-01 dated

March 30,

1989,

conducted

by

C. Casto

and

C. Payne during the periods April 6-8 and April 24 - May 4,

1989,

respectively.

The inspection included

a review of activities authorized for

your Turkey Point facility.

At the conclusion of the inspection,

the findings

were discussed

with those

members of your staff identified in the enclosed

inspection report.

During March 13-24,

1989, requalification examinations

were conducted at the

Turkey Po'int site in accordance

with NUREG-1021,,ES-601.

An unsatisfactory

program evaluation

was determined

when three out of six crews,

12 out of 24

operators

and two facility evaluators

were judged to be unsatisfactory

(Examination Report 50-250/OL-89-01).

In response,

a meeting with NRC

management

was held in Atlanta on March 29,

1989.

In order to provide

sufficient licensed operators for operation of one unit, FPL reconstituted

the

shift operating

crews to compensate

for the operators

who had failed the

requalification examination.

Unit 3 was shut

down to permit accelerated

crew

training on the plant specific simulator and to develop

teamwork

among the

reconstituted

crews.

Their performance

was then assessed

by the licensee

and

the

NRC.

C. Casto led a team which observed

crew training and associated

evaluations

on

the simulator from April 6-8. It was found that two out of four Plant

Supervisors - Nuclear

(PSN)

had difficulties in properly classifying events in

accordance

with the Emergency Plan Implementing Procedures

(EPIP).

The

facility identified weaknesses

in the Emergency Classification Table of the

EPIP's

as the root cause of this problem.

Modifications were

made to the

table

and the operators

retrained in their use.

From April 24-26,

C. Payne

observed

the upgraded

crew training and associated

evaluations

on the

simulator with emphasis

on Emergency Plan classification activities.

The

operators

were able to adequately

perform their duties including proper

classification of events

by the

PSN.

Within the scope of the inspection,

no

violations or deviations

were identified.

In accordance

with Section 2.790 of the NRC's "Rules of Practice," Part 2,

Title 10,

Code of Federal Regulations,

a copy of this letter and its enclosure

will be placed in the

NRC Public Document

Room.
