SUMMARY
- TURKEY POINT NUCLEAR PLANT - DOCKET NOS. 50-250 AND 50-251 This letter refers to the Enforcement Conference held at our request at the Turkey Point Nuclear Plant site on July 29, 1987.
The meeting concerned two violations in the areas of:
loss of boric acid flowpath and auxiliary feedwater system inoperability due to isolation of the safety-related nitrogen supply.
Enclosure 1 is an outline of the meeting topics.
Enclosure 2 is a
list of attendees.
It is our opinion that this meeting was beneficial, and provided a better understanding of the enforcement issues.
In accordance with Section 2.790 of the NRC's Rules of Practice, "Part 2, Title 10, Code of Federal Regulations,"
a copy of this letter and the enclosures will be placed in the NRC's Public Document Room.

ENCLOSURE' Enforcement Conference July 29, 1987 This meeting convened at 1:00 p.m.,
and included discussions on two events for which violations are being considered for escalated enforcement.
The first event involved the loss of a boric acid (BA) flowpath due to a failed boric acid pump mechanical seal which allowed the intrusion of nitrogen into the system and subsequent binding of the pumps.
The licensee submitted Licensee Event Report (LER) 87-17 and the potential violations are documented in Inspection Report No. 50-250,251/87-28.
The second event involved the improper valving out of both trains of backup nitrogen supply to the auxiliary feedwater trains by a non-licensed operator.
This event is discussed in Inspection Report 50-250,251/87-33.
Loss of Boric Acid Flow ath The Operations Superintendent, presented the licensee's position with regard to sequence of events, performance of operations personnel, analysis of some issues raised in Report 87-28 and the use of Event Response Teams (ERT's).
A new theory was presented concerning when and how the BA pump seal failed thus allowing the intrusion of Nitrogen.
The licensee stated that this theory could explain discrepant readings on the Auxiliary Operator's (AO) logsheets but allowed that many questions remain unanswered and that further investigation was warranted.
There was considerable discussion concerning the system engineer and non-licensed operator performing valve manipulations on the BA system.
These manipulations were apparently done without following approved procedures, adequate documentation, and knowledge and consent of control room licensed personnel.
The licensee stated that the ERT formed for this event is still in effect and that a
Human Performance Evaluation Study (HPES) is being performed.
A Mechanical
- Engineer, JPE, presented the licensee's position with respect to potential design control violations.
New information was presented concerning the design basis of the BA pump seal cooling system, their compliance with internal engineering procedures and requirements, and their response to issues raised in Report 87-28.
Ino erable AFM Trains The Operations Supervisor described the sequence of events, the licensee's analysis of the event, and their corrective actions.
Considerable effort has already been devoted to the performance of an HPES.
The licensee acknowledged that a major deficiency was that the operator manipulated the Nitrogen bottle supply valves without following approved procedures and without proper notification of supervisory personnel.
Contributing causes to this event were an inadequate training brief of a recent modification to the method of system operation; poor equipment labeling, and vague recordkeeping logs which lacked

acceptance criteria in certain areas.
The licensee acknowledged that the individual was disciplined and management emphasis was placed on procedural adherence.
In response to an NRC question, FPL does not believe that the two events are connected by a common theme of plant personnel performing operations without following approved procedures.
In the case of the first event, it was attributed to a non-licensed
>operator following the directions of a
system engineer who had exceeded his bounds of authority.
In the second event, it was attributed to a non-licensed operator attempting to correct what was believed to be a valve misalignment.
The licensee is continuing investigations into both events.
