facility.

Our preliminary findings were discussed

with Mr. J.

K. Hays, Plant

Manager, at the conclusion of the inspection.

Areas

examined

during the inspection

and our findings are discussed

in the

enclosed

inspection report.

Within these

areas,

the inspection consisted of

selective

examinations

of procedures

and representative

records,

interviews with

personnel,

and observations

by the inspector.

During the inspection, it was found that certain activities under your license

appear

to violate

NRC requirements.

These

items

and references

to pertinent

requirements

are listed in the Notice of Violation enclosed

herewith

as

Appendix A.

Elements

to

be included in your response

are delineated

in

Appendix A.

This also confirms the telephone

conversation

on November 6, 1981,

between

Mr. H.

C.

Dance, Chief, Reactor Projects

Section

2C, of our office and

Mr. K. N. Harris, Assistant

Manager,

Nuclear Energy, of your staff.

This

conversation

expressed

our concern

over the plugging of the boron injection tank,

inlet pi ping.

This concern relates

to the conditions or circumstances

that

caused this event

and the adequacy

and

scope of your corrective actions

taken to

prevent recurrence.

Consequently,

your reply should describe

in particular those

actions

taken or planned to improve the effectiveness

of your management

controls

in these

areas.

We have

examined actions

you have taken with regard to previously reported

unresolved

items.

The status of these

items is discussed

in the enclosed

report.

In accordance

with 10

CFR 2.790(a),

a copy of this letter and the enclosures will

be placed in the

NRC's Public Document

Room unless

you notify this office, by

telephone,

within ten days of the date of this letter and submit written

application to withhold information contained therein within thirty days of the

date of this letter.

Such application must

be consistent with the requirements

of 2.790(b)(l).
