APPENDIX A NOTICE OF VIOLATION Florida Power and Light Company Turkey Point 3 and 4 Docket Nos. 50-250 and 50-251 License Nos.

DPR-31 and DPR-41 As a result of the inspection conducted on October 13-16,

1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,

1980),

the following violations were identified.

Technical Specification

6. 11 requires that procedures for personnel radia-tion protection be consistent with the requirements of 10 CFR Part 20 and be
approved, maintained and adhered to for all operations involving personnel radiation exposure.

Contrary to the above, on October 14, 1981, radiation protection procedures were not adhered to in that:

( 1)

Four self-contained breathing apparatus units had not been inspected monthly as required by Plant Procedure HP-90; (2)

Four emergency respirators had not been inspected monthly as required by Plant Procedure HP-90; (3)

An inventory of emergency equipment had not been performed annually as required by Plant Procedure HP-90; and (4)

One hundred and fifty respirators had not been properly stored as required by Plant Procedure HP-60.

This is a Severity Level V, Violation (Supplement IV).

Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:

( 1) admission or denial of the alleged viola-tions; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown.

Under the authority of Section 182 of the Atomic Energy Act of

1954, as amended, this response shall be submitted under oath or affirmation.

The responses directed by this Notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
