Donald C. Cook Nuclear Plant Units 1 and 2 SPILL NOTIFICATION AND UPDATE TO PREVIOUS SPILL NOTIFICATION Indiana Michigan Power Company is the holder of National Pollutant Discharge Elimination System (NPDES) permit number MZ0005827 for Donald C. Cook Nuclear Plant. Section 5.4.2 to part IZ of appendix B (environmental technical specifications) to facility operating licenses DPR-58 and DPR-74 requires that reports of events that are reportable to other agencies be provided to the NRC.

In a letter dated August 28, 1998, the Michigan Department of Environmental Quality (MDEQ) was notified of a spill of boiler water containing hydrazine and ethanolamine. Zn a separate letter dated August 28, 1998, MDEQ was notified of an update regarding an earlier spill of Mobil DTE 797 oil. Zn accordance with the environmental technical specifications, copies of the notifications are provided to the NRC.

Donald C. Cook Nuclear Plant NPDES Permit No. MI0005827 As required by NP DES Permit MI0005827, Part II. Section A.6. Spill Notification, this written notification is being provided.

During a routine review of the alternate heating boiler logbooks on August 18, 1998, environmental personnel discovered that a spill had'occurred during an equipment drain at the site. On August 1, 1998, plant personnel were preparing the alternate heating boiler for maintenance. This involved draining the boiler contents to Outfall.OOC the plant forebay. The boiler contents were analyzed for suspended solids prior to draining for NPDES compliance. At 1350, Operations personnel began to drain approximately 4,000 gallons of water to Outfall 00C. A small leak in the blowdown drain line was noted during the drain. The boiler water from this leaked to an isolated catch basin used for chemical unloading. The drain was stopped and the supervisor was notified. Due to a misunderstanding between Operations and Chemistry Departments, a Operation's supervisor allowed the drain to continue without notifying the Environmental Department. This allowed approximately 100 gallons of heating boiler water to drain into to the chemical unloading area catch basin. The boiler water contained approximately 5.2 ppb hydrazine and 9.9 ppm ethanoiamine.

Environmental technicians, unaware of the spill, drained the catch basin twice during the week of August 3-7 after heavy rainfalls to the storm drain system. The drained storm water is routinely'ested for pH and visual observation for oil sheen, turbidity, color, foams or settleable solids. The rainfall during the next week diluted the spill as evidenced by the pH of the sample falling within the limits of 6.0 to 8.5, normal pH of heating boiler water would be >9.8. We do not believe there was any harm to the environment due to the low levels of the chemicals involved.

We are currently investigating preventive actions that can be implemented to prevent

'uch an event from occurring again.

Donald C. Cook Nuclear Plant NPDES Permit No. MI0005827 The purpose of this letter is to provide you with an update regarding our June 25, 1998 spill notification letter.

It was initially estimated that'approximately 780 gallons of Mobile DTE 797 oil was spilled to Lake Michigan over a 24-hour period. During the follow-up investigation of the incident, we estimate the duration of the release to be less than 20 hours2.314815e-4 days <br />0.00556 hours <br />3.306878e-5 weeks <br />7.61e-6 months <br />, from late June 16 to June 17. Our revised estimate of the quantity of non-hazardous, Mobil DTE 797 lubricating oil released from the Unit Two northeast Main Turbine Lube Oil Cooler is 164 gallons. Of that quantity, we believe that less than 10 gallons were discharged to Lake Michigan.

Based on visual observations in Lake Michigan, associated wind direction and shoreline evaluation, the total amount of oil recovered from the Unit Two's discharge vault, and the total amount of oil recovered from the spill, we believe our current release estimates to be more accurate than originally reported.
