D. C. COOK INSPECTION REPORT 50-315/98015(DRP); 50-316/98015(DRP)
Dear Mr. Sampson:
On July 16, 1998, the NRC completed an inspection at your D. C. Cook Units 1 and 2 reactor
facilities. The inspection was an examination, of activities conducted under your license as they
relate to compliance with the Commission rules and regulations and with the conditions with your
license.
Areas reviewed included Operations, Maintenance, Surveillance, Engineering, 'and Plant
Support.
Within these areas, the inspection consisted. of selective examinations of procedures
and representative
records, interviews with personnel, and observations of activities in progress.
The enclosed report presents the results of that inspection.
Overall, steady progress was being made in the area of repairs to the ice condensers
in both
Units. Your staff observed problems with the command and control of the ice condenser projects
and in a proactive effort, assigned additional project management
in an effort to resolve
command and control issues.
Licensed operators were also observed by the inspectors to
question the non-conservative resetting of a Limiting Condition for Operation (LCO) time clock.
In addition, prompt and extensive corrective actions were taken by your staff in response
to the
inadvertent release of carbon dioxide in the auxiliary building. The release occurred due to poor
planning associated
with a maintenance activity that had the potential to impact personnel safety.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter, the
enclosures,
and your response will be placed in the NRC Public Document Room.
