INTEGRATED INSPECTION
REPORT 50-315/316-96006(DRP)
Dear Mr. Fitzpatrick:
On July 13,
1996, the
NRC completed
an inspection at your D. C.
Cook

reactor facilities.
The enclosed
report presents
the results of that
inspection.
As a result of our inspection activities during the 6-week period covered
by
this inspection report,
we are concerned
about the two violations of NRC
requirements that were identified.
In the first violation,
NRC inspectors
identified three
examples of untimely performance of prompt operability
evaluations.
In the second violation,
NRC inspectors identified four examples
of untimely corrective action.
The second violation resulted
from issues
raised
by the Integrated
Plant
Assessme'nt
Program
(IPAP) inspection
documented
in inspection report 50-
315/96003;
50-316/96003.
The
IPAP team identified that corrective action
programs
were not being rigorously implemented.
This is
a significant concern
since the identification and resolution of deficiencies is intrinsic to the
continued safe operation of the plant.
During this inspection period
we developed
concerns with regards to your
ability to perform quality operability assessments
in a timely manner.
In
addition,
we were concerned that the operability assessments
performed
as
a
part of the large bore piping reconstitution
program did not comply with NRC Generic Letter 91-18.
Please
respond within 60 days with your justification
for your noncompliance with GL 91-18.

specified in the enclosed
Notice when preparing your response.
In your
response,
you should
document the specific actions
taken
and
any additional
actions
you plan to prevent recurrence.
Your response
may reference
or
include previous docketed
correspondence, if'the correspondence
adequately
addresses
the required response.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspections,
the
NRC will determine
whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
