INTEGRATED INSPECTION

REPORT 50-315/316-96002(DRP)

AND NOTICE OF VIOLATION

Dear

Mr. Fitzpatrick:

This refers to the routine safety inspection

conducted

by Messrs.

B. Bartlett,

D. Hartland,

C. Orsini,

and

C. Osterholtz of this office from January

17,

1996, through February

26,

1996.

The inspection

included

a review of

activities at your Donald C.

Cook Nuclear Plant,

Units

1 and 2.

At the conclusion of the inspection,

the findings were discussed

with those

members of your staff identified in the enclosed

report.

Areas

examined during the inspection

are identified in the report.

Within

these

areas,

the inspection consisted of a selective

examination of procedures

and representative

records,

observations,

and interviews with personnel.

This

report represents

an integrated

product of a number of inspections

conducted

during the inspection period.

The summary contained

in the report provides

an

overview of our assessment

of your performance

in the functional

areas

inspected.

Based

on the results of this inspection,

the

NRC has determined that

two'iolations

of NRC requirements

occurred.

The violations are cited in the

enclosed

Notice of Violation (Notice)

and the circumstances

surrounding

them

are described

in detail in the subject inspection report.

The violations are

of concern

because

they represent

a failure to recognize

the requirement to

meet

a regulation for criticality monitoring of new reactor fuel

and the

failure to ensure

a temporary modification did not represent

an unreviewed

safety question prior to installation.

In addition,

we continue to observe

problems regarding

inadequate

safety

evaluations,

improper implementation of Technical Specification action

statements,

foreign material

exclusion,

secondary

plant material condition,

the quality of auxiliary operator

rounds,

procedural

adherence,

and

human

performance.

These

problems

have

been discussed

in previous inspections.

During

a management

meeting

on January

11,

1996, your staff described

the

programs that you had

implemented to improve

human performance

and to resolve

the problems

observed

by the

NRC.

We plan to monitor these activities during

future

NRC inspections.
