Dear Sir:
The NRC was notified of a potentially. reportable deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.
Due to the extensive investigation and evaluation required, an interim report is attached. It is now expected that this information will be finalized by May 1, 1981, at which time a complete report will be sub-mitted.

0 INTERIM REPORT POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)
PVNGS UNIT it/1 I. Potential Problem While performing ultrasonic pre-service examinations in accordance with ASME Section XI on 'the Safety Injection System, a large indi-cation was found in a 16" thin. wall 32 elbow identified as Spool 1-SI-194-003. This indication is 11.5" long, O.l" wide, and O.l" deep, and is attributed to lack of fusion. Documentation at PVNGS site for this spool, furnished by Pullman Power Products, indicates that the subject elbow was purchased from the Stainless & Alloy Division of Gulf & Western Manufacturing, Energy Products Group, and was previously accepted by radiographic examination. Reexami-nation by Bechtel Construction using radiographic examination con-firmed the same acceptable results as indicated by the documenta-tion provided through Pullman.
II. A roach To and Status Of Pro osed Resolution Since discovery of the noted deficiency,'everal other similar indications have been recorded by the pre-service examination personnel. A definitive analysis to determine if these indications represent a safety significant condition is currently being per-formed. The purchase order requirements of the product apparently have been met.
III. Pro'ected Com letion of Corrective Action and Submittal Date of the Final'Re 'ort The actions necessary to perform the required evaluation will be completed within sixty (60) days. A final report will be completed and issued by May 1, 1981.
