This refers to the system
based
instrumentation
and control inspection
(SBICI)
conducted
by Hr. Z. Falevits
and others of this office on August
17 through
September
28,
1993.
The inspection
included
a review of activities authorized
for your D.
C.
Cook Nuclear
Power Plant, Units
1 and 2.
This inspection
focused
on the design
and configuration of selected
instrumentation
and
control
systems
and components.
We discussed
our inspection findings with
Hr. Blind and others of your staff at the conclusion of the inspection
on
September
28,
1993.
Areas examined during the inspection
are identified in the report.
The team
assessed
the design,
implementation,
and engineering/technical
support
relative to s'elected
instrumentation
and control
( I&C) systems.
The
inspectors
selectively reviewed the setpoint
program,
design calculations,
relevant procedures,
representative
records,
installed equipment,
and
interviewed engineering
and technical
support staff.
The team concluded
the design
and operation of the
I&C systems
examined
were
generally acceptable.
During the, inspection strengths
were identified in the
engineering staff experience
level, the guidance
provided
by Instrument
Loop
Setpoint Hethodology Guide,
and the measuring
and test
equipment
program.
However, in addition to the issues
identified in the attached
Notice of
Violation, weaknesses
were identified in ILC training for corporate
engineers
and
I&C component training for plant engineers.
We are particularly concerned
about the numerous deficiencies
we identified
with your feedwater control
system temporary modification 2-93-015.
For
example,
the engineer
inappropriately referenced
an unrelated
system's
drawing
to identify the I/I converter terminations rather
than referring to the
specific I/I converter's
vendor manual.
In another
case,
by failing to
consult the vendor manual,
the engineer
neglected
to include
a required
resistor in the I/I circuit.
Your independent
modification review failed to
identify the errors.
This particular I/I converter failed in service
and
caused
a reactor trip.
The errors
and omissions identified with this
temporary modification are unacceptable
in
a modification program developed
to
assure
the facility's safe operation.
Because
of this issue's
significance
and generic implications,
you are requested
to address
the issue
in addition
to your response
to the attached

During this inspection,
certain of your activities were in violation of NRC
requirements,
as specified in the enclosed
Notice.
The first violation is of
concern
because
omissions,
errors
and discrepancies
in setpoint calculations
and design
drawings could affect equipment operability or safe
shutdown.
The
concern with the second violation is the safety
impact of engineers
taking
an
overly restrictive interpretation of the Updated Final Safety Analysis Report
in determining whether
a safety evaluation is necessary.
You are required to respond to this letter and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should document the specific actions
taken
and
any additional
actions you plan to prevent recurrence.
After reviewing your response
to this
Notice, including .your proposed corrective actions
and the results of future
inspections,
the
NRC will determine whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
We also
request that you respond
in writing to all open
and unresolved
items noted in
this report.
We understand
that you are evaluating your organization's ability to provide
timely information during
NRC inspections.
We are interested
in your findings
because
of the issues direct effect on inspection efficiency and
effectiveness.
We therefore request that when your evaluation is complete
that
we meet to discuss
your conclusions.
In accordance
with 10 CFR 2.790 of the Commission's
regulations,
a copy of
this letter, its enclosures,
and your response
to this letter will be placed
in the
NRC Publ'ic Document
Room.
The responses
directed
by this letter
and the accompanying
Notice are not
subject to the clearance
procedures
of the Office of Management
and Budget
as
required
by the
Paperwork Reduction Act of 1980,
PL 96-511.
