This refers to the routine safety inspection

conducted

by J.

A. Isom,

D. J. Hartland,

and T. Tella of this office on June

16 through July 27,

1993,

of activities at the Donald

C.

Cook Nuclear Plant,

Units

1

and 2, authorized

by

NRC Operating

Licenses

No.

DPR-58

and

DPR-74,

and to the discussion

of our

findings with A. A. Blind and others of your staff at the conclusion of the

1nspect10n.

Areas

examined during the inspection

are identified in the report.

Within

these

areas,

the inspection consisted of selective

examinations of procedures

and representative

records,

interviews with personnel,

and observation

of

activities in progress.

Based

on the results of this inspection,

certain of your activities appeared

to be in violation of NRC requirements,

as specified in the enclosed

Notice of

Violation (Notice).

The violation regarding your failure to perform

a formal

safety evaluation prior to de-energizing

in the

open position

an

Emergency

Diesel Generator

(EDG)

room ventilation damper is

a concern

because it

ultimately contributed to the inoperability of the

EDG.

In addition, this

deficiency was not identified during your investigation into this event.

The violation regarding the Unit 2 East Motor-Driven Auxiliary Feedwater

(MDAFW) pump is of concern

because

your failure to repair

a "minor" pump

packing leak in

a reasonable

time eventually lead to

a condition where the

leak worsened

and caused

your East

MDAFW pump outer bearing to be seriously

degraded.

Our concern is reinforced

by the fact that prolonged inaction to

repair

a "minor" emergency

diesel

lube oil leak in mid-1992 eventually led to

inoperability of the diesel.

This was the subject of escalated

enforcement

action.

You are required to respond to this letter

and should follow the instructions

specified in the enclosed

Notice when preparing your response.

In your

response,

you should document

the specific actions

taken

and

any additional

actions you plan to prevent recurrence.

You are specifically requested

to

address

actions,you

plan to take to ensure

that maintenance

on safety-related

equipment -is not improperly deferred,

such that the equipment is seriously

degraded

or -rendered

inoperable.
