This refers to the investigation
conducted at the Donald
C.
Cook Nuclear
Plant by Hr.
C.
H. Weil,
NRC Region III, and Hr. J.
G. Del Percio,
Senior
Licensing Engineer,
American Electric Power Corporation,
during the period
July 16-30 and November
10,
1980.
The investigation
concerned
the alleged
violation of the Radiation
Work Permit System by employees
of NRC Region III.
The enclosed
copy of our investigation report identifies areas
examined during
the investigation.
Within these
areas,
the investigation consisted
of a selec-
tive examination of procedures
and representative
records,
observations,
and
interviews with personnel.
Based
on the investigation,
we conclude the
NRC employees
did not ask the
members of the plant's Radiation Protection Section if a Radiation
Work
Permit was specifically applicable for entry into the steam generators.
Likewise, the members of the plant's Radiation Protection Section did not
inform the
NRC employees
of the existence
of such
a Radiation
Work Permit.
As
a result,
the
NRC employees
did not establish that such
a Radiation
Work
Permit was posted,
consequently
the
NRC employees
did not observe
the
established
plant radiation protection procedures.
A memorandum will be
sent to all Region III inspection personnel
reminding them of their
responsibility to become familiar with and observe
the established
radiation
protection procedures
at plants they visit.
In accordance
with Section 2.790 of the NRC's "Rules of Practice," Part
2, Title 10,
Code of Federal Regulations,
a copy of this letter and the
enclosed investigation report will be placed in the NRC's Public Document
