RE(UALIFICATION EXAHINATIOH REPORT During January 30 through February 7, 1991, the HRC administered requalification examinations to employees of your organization who operate your D. C.
Cook Nuclear Plant.
At the conclusion of the examination, any generic findings that evolved as a result of the examinations were discussed with those members of your staff identified in the enclose'd report.
As a result of this evaluation, your requalification program has been assigned an overall program rating of satisfactory in accordance with the criteria of HUREG-1021, ES-601.
For those individuals with unsatisfactory
- results, the facility should take corrective action as required by its NRC approved requalification 'program.
One individual failed hi.s second retake examination.
The facility had remediated the individual twice due to the failure of the original requalification exam-ination and the failure of the first retake examination.
The failure of the individual brings into question the ability of the facility to remediate.
individuals to ensure they meet minimum requalification standards.
Please respond in writing within 20 days of the date of this letter to our corcern regarding the above.
Your response should indicate why this individual was not successfully r emediated and your plans to correct any related deficiencies in your program.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter, the enclosures, and your response will be placed in the NRC Public Document Room.
