$990 This letter is being issued to publicly release the Office of Investigation synopsis for OI Case Number 3-87-018 (enclosed) regarding a quality control inspector who allegedly falsified a safety related fire extinguisher record.
This item was previously addressed as a Severity Level IV Violation issued in Inspection Report Number 50-315/87017; 50-316/87017.
This completes all NRC actions regarding this issue (AMS NO. RIII-87-A-0129).

SYNOPSIS On October 19, 1987, the NRC Region III Administrator requested that the Office of Investigations (OI) investigate an alleged falsification of a safety-related record by an employee at the Donald C.
Cook Nuclear Power Plant (Cook).
The OI investigation has determined that on February 28,
- 1987, a Cook quality control inspector knowingly falsified a monthly fire extinguisher inspection checklist.
That individual made a conscious decision,to falsify the record without direction or pressure from his supervision.
However, that falsification was not performed in any way to either maliciously impede or degrade fire safety or the safe operation of the Cook plant.
The individual responsible for the falsifications perceived that the circumstances surrounding his decision, although technically wrong, warranted that action.
The quality control inspector who violated the NRC regulations by making a
false record was ultimately fired by Cook management for his actions, even though he was only eight months away from becoming vested in the utility's retirement program.
