A. Gavula of this office from July

1989, of activities

at the D. C.

Cook Nuclear

Power Station, Units

1 and 2, authorized

by NRC Operating

Licenses

No.

DPR-58 and

No. DPR-74

and to the discussion

of our findings with

Hr. D. Williams and others at the conclusion of the inspection.

The enclosed

copy of our inspection report identifies areas

examined during

the inspection.

Within these

areas,

the inspection consisted

of a selective

examination of procedures

and representative

records,

observations,

and

interviews with personnel.

The focus of this inspection

was the review of

inservice inspectior, activities for both Units

1 and

2 and the review of

specific actions

taken in response

to

NRC Violations regarding

design control.

During this inspection, certain of your activities appeared

to be in violation

of NRC requirements,

as specified in the enclosed

Notice.

A written response

is requ'ired.

With respect to previous Violations 315/88028-06;

316/88032-06,

please

submit to this office within thirty days of the date of this letter

a

written statement

providing a plan of action to ensure that adequate

weld size

was installed during previous modification work.

The design control violation cited in this report stems

from the review of

structural modifications performed since the issuance

of the design control

violation in NRC Inspection Reports

No. 50-315/88028

and

No. 50-316/88028.

Although we recognize that you have

a comprehensive

long term corrective action

program that is in the infancy of implementation, it appears

that your interim

actions that included in-depth discussions

with designers/engineers

on

expectations

of high quality work have not been totally successful.

Your

written response

to this violation should address

the root cause of this

continuing problem, the reasons for the ineffectiveness

of your previous

interim corrective action,

and describe further action you are taking to ensure

a quality engineering/design, product for future modifications across all

disciplines.
