EXAMINATION REPORT On July 10-19,
- 1989, the NRC administered examinations to employees of your organization who had applied for licenses to operate your D.C.
Cook Nuclear Power Plant.
At the conclusion of the examination, the examination questions and preliminary findings were discussed with those members of your staff.

The simulator portion of the examinations conducted r'esulted in a 35% failure rate.
The region conducted a followup inspection during the we'ek of August 14, 1989, to determine the root cause of these failures.
The inspection findings concluded that:
Instructors served as evaluators thereby limiting objectivity.
Facility audits did not test the students to the degree of an NRC exam thereby limiting the scope of their evaluation.
Individual weaknesses were not compared to determine possible programmatic weaknesses thereby overlooking a generic weakness in procedure usage.
Details of the inspection are documented in paragraph 3 of this report.

We also request that you discuss your strategy to prevent a recurrence of such a high failure rate.
