Dear Mr. Kilmer:
On April 5, 1989 at 2:40 p.m., personnel at the Indiana Michigan Power Company (I&M)
Cook Nuclear Plant discovered a
small leak in one of Unit 2's main turbine lubricating oil coolers.
Lake water from the nonessential water system is pumped through cooler tubes to cool the lube oil.
This small hole created a
connection between the cooler and the nonessential water system which discharges to Lake Michigan.
The leaking cooler was isolated on April 5, 1989 thereby preventing any more loss of oil to the nonessential water system.
The exact amount of oil lost is not
- known, but at the time the cooler tube leak was discovered, the same system experienced an internal leak which accounted for a majority of the initially reported 635 gallons.
It is now estimated that the amount of oil that reached Lake Michigan was less than 100 gallons, based on leak rate testing conducted after the cooler was isolated.
An oil and grease analysis performed on a
sample taken from the Unit 2 discharge sample point indicated less than detectable values.
A physical inspection of the shoreline and nearshore waters revealed no oil present.
The incident was reported by telephone to the National Response Center, report 04941 and the Michigan PEAS on April 5, 1989.
In accordance with 40 CFR 112.4, a detailed report is being prepared to address this incident and the incident of February 24, 1989.
