Indiana and Michigan Electric Company's letters of May 16 and October 10, 1986, and May 28 and November 10, 1987 pertaining to Cook Units 1 and 2, contained your response to IEB 85-03, "Motor-Operated Valve Common Mode Failures During Plant Transients Due to Improper Switch Settings".

Our review of the responses indicates the need for additional information before the program to assure valve operability can be approved.

The specific information necessary for the completion of our review is enclosed.

Additional information is requested within one month of the date of this letter.

The written report shall be submitted to the Regional Administrator, Region III, under oath or affirmation under provisions of Section 182a, Atomic Energy Act of 1954, as amended.

Also, the original copy of the cover letter and a copy of the report shall be transmitted to the U.S. Nuclear Regulatory Commission Document Control Desk, Mashington, DC 20555 for reproduction and distribution.

Vice President The information provided in your response to Action Item e of IE Bulletin 85-03 was found to be deficient in some areas.

Provide the additional information necessary to resolve the following comments and questions:

1.

Has water hammer due to valve closure been considered in the determination of pressure differentials?

If not, explain.

2. If MOVATS is planned for application to some MOVs which are not included in its data
base, commit to and describe an alternate method for determining the extra thrust necessary to overcome pressure differentials for these valves.
3. Unlisted cold leg injection MOVs IM0-51, -52, -53 and -54 (1 1/2-inch) are normally open downstream of BIT outlet valves ICM'-'250 and -251.

These MOVs are shown in zones E-2, D-'2, C-2 and F-2 respectively

-on drawings 1-5142-24 (Unit 1) and 2-5142-28 (Unit 2).

How would safety injection to 'the cold legs be ensured if these MOVs were to be left closed inadvertently?

Revise Table I of the response of 05-16-86 to include these MOVs, or justify their exclusion.

4.

The proposed program for action items b,

c and d of the bulletin is incomplete.

Provide the following details as a

minimum:

(a) commitment to a training program for setting switches, maintaining valve operators, using signature testing equipment and interpreting signatures, and (b) commitment to justify continued operation of a valve determined to be inoperable.
