3 through March 3, 1986, of activities at D. C.
Cook
Nuclear
Power Plant Units
1 and 2, authorized
by

It also refers to the discussion
of our findings
with Mr.
W. G. Smith and members of his staff at the conclusion of the onsite
inspection
on February
14,
1985,
and further discussions
between
our staffs
through March 3,
1986.
The enclosed
copy of our inspection report identifies areas
examined during
the inspection.
Within these
areas,
the inspection
consisted of a selective
examination of procedures
and representative
records,
observations,
and
interviews with personnel.
During this inspection,
certain of your activities appeared
to be in violation
of NRC requirements,
as specified in the enclosed
Notice.
A written response
is required.
The purpose of this inspection
was to determine
the adequacy of your design
change
program implementation.
We found the installation of completed
design
changes
to be generally well controlled.
Although our inspectors
identified
one instance
where field changes
were improperly controlled
and another where
plant modifications were not properly documented
in the engineering
drawings,
they appeared
to be isolated
cases.
In contrast,
we have significant concerns
regarding your corrective action program
as it relates
to plant modifications.
Specifically, the promptness
and correctness
with which you identify root causes,
implement corrective measures
and assign priority to plant modification work
needs
to be given greater attention.
These
issues
are reflected in Violation
No.
2 of the Appendix and discussed
further in Paragraphs
2.b.(8)
and 2.b.(12)
of the attached
inspection report.
The issue of untimely and inadequate
corrective action
has
been longstanding
at D.
C. Cook.
We will contact you in the near future to schedule
a meeting
to discuss
both your response
to this report and your broader
proposal
to
address
the apparent
shortcomings
in your current corrective action program.
