Notice of Violation American Electric Power Service Corporation Docket No. 50-315 Docket No. 50-316 As a result of the inspection conducted on January 21, 1986 through February 18, 1986 and in accordance with the General Policy and Procedures for NRC Enforcement Actions (10 CFR Part 2, Appendix C), the following violations were identified:

Units 1 and 2 Technical Specifications 6.8.2 and 6.8.3 permit changes to procedures of Specification
- 6. 8. 1, which includes procedures for surveillance and test activities of safety related equipment, provided the change is reviewed by the Plant Nuclear Safety Review Committee (PNSRC) and approved by the Plant Manager prior to implementation (for permanent changes);
or, provided the change does not alter the procedure's intent, is approved by two members of management (one with an SRO license), is documented, and is reviewed by the PNSRC and approved by the Plant Manager within 14 days of implementation (for temporary changes).
Contrary to the above, on August 22, 1985 an NRC inspector observed licensee personnel performing a test of safety related equipment using a
procedure (2 THP 4030 STP. 145, "Reactor Logic Train A and B and Reactor Trip Breakers A and B") which had been changed by handwitten revisions, and these changes had not been subjected to either type of required review, approval and documentation process.
Further review indicated handwritten changes were relatively common in procedures of this type.
This is a Severity Level IV violation (Supplement I).

Unit 2 Technical Specification 3.3.3. 10b requires, with less than the minimum number of radioactive gaseous effluent monitoring instrumentation channels OPERABLE, take the ACTION shown in Table 3.3-13.
The ACTION shown in Table 3.3-13 for no OPERABLE Unit auxiliary building ventilation system noble gas activity monitor is ACTION 28, which permits continued effluent releases provided grab samples are taken at least once per eight hours.
Contrary to the above, on December 27, 1985 with no OPERABLE Unit 2 auxiliary building ventilation noble gas activity monitor, ACTION 28 was not complied with in that consecutive samples were taken after an interval of 10 hours1.157407e-4 days <br />0.00278 hours <br />1.653439e-5 weeks <br />3.805e-6 months <br /> and 22 minutes rather than at least once per eight hours.
This event appears repetitive of previous occurrences on June 3, 1985
- and, on Unit 1, on November 8 and 12, 1984.
This is a Severity Level IV violation (Supplement I).

Notice of Violation With respect to item 1., the inspection showed that action had been taken to correct the identified violation and to prevent recurrence, consequently, no reply to the violation is required and we have no further questions regarding this matter.
With respect to Item 2., pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation: (1) corrective action and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when f'ull compliance will be achieved.
Con'sideration may be given to extending your response time for good cause shown.
