A endix A NOTICE OF VIOLATION American Electric Power Service Corporation Indiana 6 Michigan Electric Company Docket No. 50-315 Docket No. 50-316 Based on the inspection conducted from April 1, 1980 to June 15, 1980, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.

Items 1 through 3 are infractions; Item 1

is applicable to both units and Items 2 and 3 are applicable to Unit 2 only.

1.

Technical Specification 6.2.2, Table 6.2-1, requires 6 licensed

, operators for minimum shift composition in modes 1, 2, 3 and 4.

Contrary to the above, on the 0001-0800 shift of April 5 and 6,

1980, only 5 licensed operators were present with both units in mode 1.

2.

Technical Specification 6.8.1, requires procedures to be developed, implemented, and maintained.

OHI-2211 requires significant shift activities to be logged in both the applicable Control Room logbook and the Shift Operating Engineer logbook.

Contrary to the above, on May 10, 1980, Unit 2 was inadvertently taken to 101/, power for several minutes

{exceeding the license limit) due to a valving error with resultant dilution.

Neither the valving error nor exceeding 100/ power were noted in either logbook.

Technical Specification 3.7.1.2 action statement for auxiliary feedwater pumps requires an inoperable pump be returned to service within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or proceed to hot shutdown.

Contrary to the above, on May 27, 1980, it was discovered that the Unit 2 turbine driven auxiliary feedwater pump had exceeded the referenced action statement due to insufficient review and approval of a surveillance test that had identified parameters which were not within the operability acceptance criteria on May 20, 1980.
