MOTOR OPERATED AUXILIARY FEEDWATER VALVES WAS REMOVED FROM SERVICE FOR ROUTINE INSPECTION.

IN GRANTING CLEARANCE ON THE MOTOR CONTROL CENTER IT WAS PROPERLY NOTED ON THE FORM THAT THE TWO VALVES WOULD BE HADE INOPERABLE BY THAT ACTION.

CAUSE

DESCRIPTION AND CORRECTIVE ACTIONS:

DRIVEN AFP WAS ALSO INOPERABLE.

FIVE MINUTES AFTER THE VALVES WERE DE-ENERGIZED HE DISCOVERED HIS ERROR AND IMMEDIATELY TOOK STEPS TO HAVE THE POWER RESTORED.

THE VALVES WERE INOPERABLE FOR A TOTAL OF 12 MINUTES.

A STATUS BOARD HAS BEEN PROVIDED TO SIMPLIFY THE TRACKING OF THE OPERABILITY STATUS OF THE AUX FEEDWATER SYSTEM OF BOTH UNITS.
